EPIC OUTREACH, INC., Waiver and Release of Liability

• In consideration of visiting this farm and interacting with the animals provided by Epic Outreach, Inc., I expressly agree and contract, on behalf of myself, my heirs, executors, administrators, successors and assigns, that Epic Outreach, Inc. and its insurers, employees, officers, directors, property owners, and associates (collectively "Epic Outreach, Inc.”), shall not be liable for any damages arising from personal injuries (including death) sustained by me, or my guest in, on, or about the premises, or as a result of the animals and other injuries that may arise out of interacting with them in any manner, regardless of whether such injuries result, in whole or in part, from the negligence of Epic Outreach, Inc.

• By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me or my guest, and I hereby fully and forever release and discharge Epic Outreach, Inc., its insurers, employees, officers, directors, property owners, and associates, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the visitation of this farm or interacting with the animals.

• I expressly agree to indemnify and hold Epic Outreach, Inc. harmless against any and all claims, demands, damages, rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me or my guest caused by any animal on the premises. This includes, but is not limited to horses, canines, pigs, cows, or chickens. This includes any and all claims that may arise out of damages from canines on the premises that may otherwise be specified under Fla. Stat. §767.01 & §767.04. This further includes and all claims that may arise out of damages from horses on the premises that may otherwise be specified under F.S.A. §773.01-02. 

• I agree to be solely responsible for safety and wellbeing of my guest and myself. I understand that Epic Outreach, Inc. does not provide supervision, instruction, or assistance for each and every guest that enters the farm. 

• I agree to comply with all rules imposed by Epic Outreach, Inc. regarding the use of the facilities and the farm. I agree to conduct myself in a controlled and reasonable manner at all times, and to refrain from using any equipment in a manner inconsistent with its intended design and purpose. 

• I understand and acknowledge that visiting and interacting with this farm’s animals can involves risk of serious injury, including permanent disability and death.

• I understand and agree that Epic Outreach, Inc. is not responsible for property that is lost, stolen, or damaged while in, on, or about the premises.

• I understand and agree that visitation to this farm is only to be undertaken on my own personal time, and that my use of the facilities and equipment is not within the course or scope of my employment. 

• I agree and grant to Epic Outreach the right to take photographs and videos of me, my property, my child(ren), and/or any minor in my care in connection with visitation to this farm, and hereby allow Epic Outreach the right to use, publish, or print any photos physically or electronically any photos that may be taken of me, my property, my child(ren), and/or any minor in my care.



NOTICE TO THE PARENT OR GUARDIAN OF MINOR CHILDREN READ THIS FORM COMPLETELY AND CAREFULLY.

• You are agreeing that you are the parent or legal guardian of the child for whom you are

signing. If you are not the parent or legal guardian of the child, you have received express

written consent from the parent or legal guardian of the child to execute this release and

waiver on behalf of the child and have provided a copy of the attached written consent to

Epic Outreach with this Form.


• You are agreeing to allow your child to participate in a potentially dangerous activity. You

are agreeing that even if Epic Outreach, Inc. uses reasonable care in providing interactions

with this farm’s animals, there is still a chance your child may be seriously injured or killed

by participation in interaction with this farm’s animals because there are certain dangers

inherent in interacting with animals that cannot be avoided or eliminated.


• By signing this form, you are giving up your child’s right and your right to recover from

Epic Outreach, Inc. in a lawsuit for any personal injury, including death to your child or any

property damage that results from the risks that are a natural part of interacting with this

farm’s animals.


• You have the right to refuse to sign this form, and Epic Outreach, Inc. has the right to refuse

to let your child participate if you do not sign this form.


• This is your release and waiver of liability. You, individually and on behalf of your minor

child, effectively release Epic Outreach, Inc. immediately of any and all liability. 


I HAVE READ THE FOREGOING WAIVER AND RELEASE OF LIABILITY AND VOLUNTARILY EXECUTED THIS DOCUMENT WITH FULL KNOWLEDGE OF ITS CONTENT.
