I acknowledge that taking part in water-sports such as paddle boarding or playing on a trampoline is a potentially dangerous activity, as shown by my signature below. The action carries inherent hazards, such as serious injury and/or death. I am aware that while Altitude Surfing, LLC (collectively referred to herein with Altitude Surfing employees, owners, officers, directors, agents, independent contractors and assigns as Altitude Surfing) has taken all reasonable steps to provide a safe experience, including regular checks on equipment, providing a qualified and experienced instruction, as well as making preparations for my safety as a user of their equipment, it is still not possible for Altitude Surfing to guarantee my safety prior to, during, or after such instruction and operation of said equipment. Therefore, as a prerequisite of my participation in activities offered by Altitude Surfing, I hereby release and waive my rights to assert any claims for injuries or damages against Altitude Surfing, and its assignees, heirs, agents, employees and Contractors, including, but not limited to, Altitude Surfing.

  • I further acknowledge that, as per the date of my signature below, I freely requested and applied to take part in activities that will be provided and carried out by Altitude Surfing.
  • I'm aware that participating in water activities has the potential to be hazardous. My involvement in lake related sports carries the risk of severe bodily harm or death. Among the risks and hazards are, but are not limited to: theft or damage to personal property; injury or death, collisions with objects and other boaters; accidents or illnesses in remote locations without access to medical care; hypothermia; and subsequent drowning. With full understanding of the risks and dangers involved, I willingly agreed to participate in the instruction and operation of the aforementioned rental equipment as well as any other connected activities. I hereby agree to accept and assume all risks of damage, injury, or death, and I hereby confirm the above by signing below.
  • I am aware of my responsibility for my own safety as well as the safety of others, and I accept that responsibility. I agree to follow Altitude Surfing’s instructions before and during the operation of the aforementioned equipment. I am aware of and agree to abide by the Altitude Surfing’s Safety Policies, which are detailed below:

Safety Policies: Altitude Surfing’s top priority is the safety of our renters. Decisions are made by Altitude Surfing based on a variety of variables. These factors may include, but are not limited to: weather forecast, lake conditions such as algae blooms or pollution, and other conditions before and during the setup for the instruction and use of equipment. Altitude Surfing retains sole discretion over all choices involving safety, including whether to instruct & operate the aforementioned equipment and how long they should be used. The Rider/Passenger shall always have the option to refuse to take part in related water sports. To the best of my knowledge, I do not suffer from any physical or mental conditions that would prevent me from participating safely in the instruction and operation of such equipment.
