You further confirm that the minor is feeling fit and well and does not know of any physical impairment which could adversely affect their ability to use the fitness equipment provided in a safe manner. This signature also indicates that you have read and understood this waiver and liability. You are aware that this is a waiver and a release of liability and you voluntarily agree to its terms.<br><br> By signing this agreement you acknowledge that you have received and read a copy of the Revo Fitness membership agreement, membership liability release and general terms and conditions fund within this document. By signing this agreement you agree to accept the terms of this agreement in its entirety and to be bound by the conditions for the period of my membership. <br><br> By signing this agreement you understand that you have given consent to an underage person to use the facility, Revo Fitness. Revo Fitness takes no responsibility for any incidents at any Revo Fitness Location. You agree to all terms and conditions that Revo Fitness has for the underage user of Revo Fitness. <br><br> I acknowledge that Revo Fitness does not operate Revolution Sports (37 Lemnos Street, Shenton Park WA 6008) and is not responsible, and accepts no liability, for the other facilities at Revolution Sports, including the café and change rooms.
