Dainty Links Permanent Jewelry Waiver & Release

Thank you for choosing Dainty Links, a service provided by Pretty Little Face LLC. Please read and sign the following waiver before your appointment.


1. Acknowledgment of Procedure

  •  I understand that permanent jewelry involves welding a custom-fit chain directly onto my wrist, ankle, or neck.

  •  I understand that while the jewelry is designed to be long-lasting, it can be removed by cutting the chain if desired.


2. Materials & Allergies

  •  Dainty Links uses high-quality materials including gold-filled, sterling silver, and stainless steel.

  •  I understand that all metals may naturally tarnish, dull, or cause skin reactions in rare cases.

  •  I release Pretty Little Face LLC, doing business as Dainty Links, from liability for any allergic reaction or skin irritation that may occur.


3. Risks & Limitations

  •  I understand permanent jewelry may snag, break, or stretch if pulled.

  •  I acknowledge that activities such as sports, medical procedures (like MRIs or surgeries), or certain jobs may require removal of my jewelry.

  •  I understand that once cut or removed, the jewelry cannot be re-welded if the chain is damaged.


4. Care & Responsibility

  •  I understand I am responsible for proper care of my jewelry.

  •  Pretty Little Face LLC / Dainty Links is not responsible for lost, broken, or damaged jewelry after my appointment.


5. Waiver of Liability

  •  I release Pretty Little Face LLC and its employees, operating as Dainty Links, from any responsibility for injury, allergic reaction, damage, or loss related to my permanent jewelry.

  •  I consent to receiving this service and understand the nature of the procedure.
