Participation Waiver

PARTICIPANT WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT

This Agreement must be completed and signed by all participants. For participants under the age of 18, a parent or legal guardian must also sign.

For purposes of this Agreement, “Facility” refers to Edge Sports Global LLC and its managed and affiliated properties, including but not limited to Albany Road – North Smithfield LLC, Albany Road – Wellesley LLC, Albany Road – Foxboro LLC, Albany Road – Boch LLC, Albany Road – Thayer LLC, Albany Road – Bridgewater LLC,  Albany Road – Pawtucket LLC, Albany Road – Rockland LLC, Albany Road – Canton LLC, and their respective owners, officers, employees, agents, and affiliates (collectively, the “Releasees”).


1. ASSUMPTION OF RISK

I understand that participation in sports, recreation, and related activities (the “Activities”) at the Facility—including but not limited to ice rinks, courts, turf fields, tracks, fitness areas, and aquatic/pool environments—involves inherent risks.

Falls, slips, trips, and collisions

Contact with other participants, staff, or spectators

Contact with boards, glass, walls, flooring, turf, equipment, or structures

Surface conditions (including ice, water, turf, court surfaces, or track conditions)

Equipment failure or improper use

Exposure to water-related risks, including drowning or slip hazards in aquatic areas

Weather-related or environmental conditions (indoor or outdoor)

Acts or omissions of other participants or staff

Serious bodily injury, illness, permanent disability, or death

I acknowledge that these risks may arise from my own actions, the actions of others, the condition of the Facility, or the negligence of the Releasees.

I voluntarily and knowingly assume all such risks and accept full responsibility for my participation.

2. RULES, SAFETY, AND CONDUCT

I agree to:

Follow all Facility rules, policies, and posted guidelines

Use required protective equipment where applicable

Act in a safe and respectful manner at all times

If I observe unsafe conditions, I will immediately stop participation and notify Facility staff.

3. RELEASE OF LIABILITY

To the fullest extent permitted by law, I release and hold harmless the Releasees from any and all claims, demands, or causes of action arising out of or related to my participation in the Activities, including claims arising from the negligence of the Releasees.

4. INDEMNIFICATION

I agree to indemnify and hold harmless the Releasees from any claims, damages, or expenses (including reasonable attorney’s fees) arising from:

My participation in the Activities

My actions or failure to follow Facility rules

Claims brought by or on behalf of a minor participant

5. MEDICAL AUTHORIZATION

I authorize the Facility to provide or arrange for basic first aid and emergency medical care if necessary.

I understand that:

The Facility does not guarantee medical services

I am solely responsible for all medical expenses incurred

6. MEDIA RELEASE

I grant permission for the Facility to use the participant’s name, image, and likeness in promotional materials, including websites and social media, without compensation.

7. ARBITRATION

Any disputes arising out of participation in the Activities shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association.

Arbitration shall take place in the state where the Facility is located and must be initiated within one (1) year of the claim arising.

8. SEVERABILITY

If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

9. ACKNOWLEDGMENT

I have read this Agreement, fully understand its terms, and understand that I am giving up certain legal rights by signing it. I sign this Agreement freely and voluntarily.

PARENT / GUARDIAN CONSENT (REQUIRED FOR MINORS)

I am the parent or legal guardian of the above-named minor. I have read and understand this Agreement and consent to the minor’s participation.

I acknowledge that I am responsible for the minor’s conduct and safety and agree to the terms of this Agreement on their behalf.
