DRIBBL WAIVER

I hereby allow my child to participate in the Dribbl basketball program.

I understand that there are certain risks of injury inherent in the practice and play of this sport, as well as in traveling and other related activities incidental to my child's participation, and I am willing to assume these risks. I hereby certify that my child is fully capable of participating in the Dribbl program and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict a full participation in these activities.

In addition to giving full consent for my child's participation, by clicking "I Agree" below, I do hereby waive, release and hold harmless the Dribbl program, its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the program and the activities incidental thereto, whether the result of negligence or any other cause.

AUTHORIZATION TO USE PHOTOGRAPHS AND/OR AUDIO-VISUAL

I hereby authorize Dribbl Basketball to use, reproduce, and/or publish photographs and/or video that may pertain to my child including his or her image, likeness and/or voice without compensation. I understand that this material may be used in various publications, public affairs releases, recruitment materials or for other related endeavors. This material may also appear on Dribbl's website. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Consequently, Dribbl may publish materials, use my child's name, photograph, and/or make reference to him or her in any manner that Dribbl deems appropriate in order to promote/publicize service opportunities."
