WAIVER OF PUBLICITY FORM

I give permission for the use of any photos, audio, or video tapings of my students’ activities at events in connection with Vineyard Community Church youth ministry, to be used with VCC’s approval for religious purposes, media coverage, or for publicity benefiting religious purposes.
