MDScootering Waiver and Release

This form must be completed and signed by you before you are permitted to participate in events or lessons run by MDScootering. If you are under 18 years of age a parent or legal guardian must sign this form and provide an emergency contact number.

BY COMPLETING THIS FORM I AM AGREEING THAT MY/MY CHILDS PARTICIPATION IN MDSCOOTERING ACTIVITIES/EVENTS AND LESSONS ARE SUBJECT TO THE BELOW TERMS AND CONDITIONS AND ACKNOWLEDGMENTS AND WAIVERS SET OUT IN THIS APPLICATION AND BELOW.

TERMS, CONDITIONS, ACKNOWLEDGMENTS AND WAIVERS.

These Terms, Conditions, Acknowledgements and Waivers apply to all activities in which the Rider takes part during his/her/they/them/their lesson/event/competition with MDScootering. It is a condition to the Rider being accepted into an MDScootering lesson/event/competition that the following points be read and acknowledged by clicking submit form below. 

RISK WARNING AND EXCLUSION OF LEGAL LIABILITY

MDScootering makes safety its first priority while enjoying the fun and healthy sport of scootering however, there are risks involved in scootering as with any other sport. During a MDScootering lesson/event/competition, riders must wear a helmet and are strongly advised to wear full protective gear (ie. Knee pads, elbow pads, wrist guards, mouth guards). This form must be submitted by the Rider or a Parent/Guardian if the Rider is under 18 years of age, prior to his/her/they/them commencing activities with MDScootering and prior to he/she/they/them being entitled to participate in lessons/events/competitions with MDScootering.

Please read carefully the following acknowledgements and assumptions of risk relating to activities provided by MDScootering:

The Rider warrants that he/she/they/them is fully medically fit to undertake the lesson/event/competition.

The Rider agrees that he/she/they/them will participate in the lesson/event/competition entirely at his/her/they/them own risk.

The Rider acknowledges that the lesson/event/competition carries some risks and/or dangers including physical injury that may result in permanent disability or death, and economic loss or damage.

The Rider agrees to fully comply with all directions given by the MDScootering coaches and officials.

By participating in scootering lesson/event/competition with MDScootering and/or using equipment provided to the Rider by MDScootering, either at this time or in the future, the Rider agrees:

That the Rider participates in activities provided by MDScootering at his/her/they/them own risk.

That the Rider agrees that in participating in a lesson/event/competition provided to him/her/they/them/they/them by MDScootering he/she/they/them totally releases from all liability whatsoever and indemnifies MDScootering and its employees and agents and contractors against any losses, damages, costs, expenses, liabilities and judgments suffered or incurred by the Rider or any other person in respect of the said activities.

That MDScootering has no liability to the Rider for personal injury, disease or death arising from the services and/or equipment that MDScootering provides to the Rider.

That the Rider will comply promptly with all directions given to him/her/they/them by coaches and other officials of MDScootering, its employees, agents and contractors during any MDScootering lesson/event/competition.

MDScootering may take photos/videos during a MDScootering lesson/event/competition and Riders agree that photos/video taken at such times may be used for promotional or marketing purposes by MDScootering.

 I, the Rider (Parent/Guardian if Rider if under the age of 18), confirm that I have read and understood and accept the above terms and conditions, acknowledgements, warranties, waivers and indemnities.

BY ACKNOWLEDGING AND CLICKING SUBMIT BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
