Adult Medical Release: Conquerors Sports Academy LLC ('CSA'), a South Carolina limited liability company, is offering and administrating an adult sports program, ('Program'). The Program will be held at First Presbyterian Church (FPC) . The Program offers a fun, Christian-centered approach to adult recreation. In consideration for my participation in the activities and events sponsored or conducted by CSA, I acknowledge and agree that: I understand there is a risk of injury from the activities involved in my participation in the Program including the potential for serious injury, permanent disability or death. I acknowledge that I have no physical or mental condition(s) that would prevent me from participating in activities or events of the Program. In the event that I am injured while participating in the Program and I am not physically responsive, I give permission to CSA and FPC, to obtain and authorize medical treatment for me at any hospital, emergency medical center, or other health facility and agree to be responsible for the expenses of any medical care obtained under this agreement. I, and anyone entitled to act on my behalf, agree to indemnify, release and forever hold harmless CSA and FPC, their affiliates, owners, officers, directors, managers, employees, members, agents, representatives, coaches, and volunteers ('Released Parties'), of and from any and all risks and liabilities, both known and unknown, including all claims of injury, disability, death or loss or damage to person or property, whether arising from negligence of the Released Parties or otherwise, which I may have or may hereafter obtain against the Released Parties related to my participation in the Program. Youth Medical Release: Conquerors Sports Academy LLC ('CSA'), a South Carolina limited liability company, is offering and administrating a youth sports program, ('Program'). The Program will be held at First Presbyterian Church (FPC). The Program offers a fun, Christian-centered approach to youth recreation. In consideration for my child being permitted to participate in the activities and events sponsored or conducted by CSA, I acknowledge and agree that: I am the parent or legal guardian of a child ('Child') participating in CSA. I understand there is a risk of injury to my Child from the activities involved in participation in the Program including the potential for serious injury, permanent disability or death. I acknowledge that my Child is in good health and has no physical or mental condition(s) that would prevent him/her from participating in activities or events of the Program. In the event that my Child is injured while participating in the Program and I am not physically present, I give permission to CSA and FPC, to obtain and authorize medical treatment for my Child at any hospital, emergency medical center, or other health facility and agree to be responsible for the expenses of any medical care obtained under this agreement. I, and anyone entitled to act on my behalf or on behalf of my Child, agree to indemnify, release and forever hold harmless CSA and FPC, their affiliates, owners, officers, directors, managers, employees, members, agents, representatives, coaches, and volunteers ('Released Parties'), of and from any and all risks and liabilities, both known and unknown, including all claims of injury, disability, death or loss or damage to person or property, whether arising from negligence of the Released Parties or otherwise, which I and/or my Child may have or may hereafter obtain against the Released Parties related to my child’s participation in the Program.
Other Disclosures: Weather or Extenuating Circumstance Cancellations: I acknowledge and accept that some events offered by CSA may be cancelled and may not be rescheduled due to weather related issues or other extenuating circumstances, and these cancellations will not result in any type of monetary refund. Image Waiver: I give permission to CSA and FPC to use images and/or video of my Child or me in all types of media for public relations purposes to promote the Program or CSA. Zero Tolerance Policy Towards Referees, Coaches and CSA Employees: I acknowledge that I will treat referees, players, other parents, coaches and CSA employees with respect by not yelling or making negative remarks. I acknowledge that I may be asked to leave the playing area by an CSA employee if I do not comply with this policy. Special Requests: I acknowledge and accept that special requests, buddy requests or teammate requests may not be honored. The goal of the league is to provide an equally balanced athletic experience for all players and special requests, especially teammate requests, can harm this experience. Logistically, it is impossible to honor all special requests with the high number of participants at CSA. Concussion Protocol: Concussion awareness education material can be obtained for free at https://www.cdc.gov/headsup/youthsports/index.html. I acknowledged that concussion awareness education materials have been made available to me by CSA.
I have read this waiver and release of liability agreement, fully understand its terms, and acknowledge that I have given up substantial rights by agreeing to the medical release and waiver.
