1. Activities Covered This agreement applies to participation in Clay Rose Pottery classes, workshops, private events, memberships, open-studio use, independent studio use, demonstrations, community events, and related ceramic, floral, craft, and creative activities on the premises. 2. Acknowledgment of Inherent Risks I understand that a working ceramics and art studio involves inherent risks, including slips and falls; wet floors; lifting clay and supplies; repetitive movement; pottery wheels and hand tools; sharp or broken ceramics; clay, dust, glaze and other art materials; skin irritation; burns or heat from kilns or fired ware; electrical equipment; shelving and stored objects; and accidental injury caused by my actions or those of other participants. Following studio procedures reduces risk but cannot eliminate every risk. 3. Studio Safety & Instructions I agree to follow posted rules, instructor directions, safety instructions, cleanup procedures, equipment restrictions, and studio policies. I will use equipment only as instructed and when authorized. I will promptly report spills, damaged equipment, unsafe conditions, or injuries. 4. Clay, Glaze, Dust & Materials I agree not to intentionally create airborne clay or glaze dust and to follow wet-cleaning and material-handling procedures. I will not use outside clay, glaze, chemicals, or other materials without studio approval. I will disclose relevant allergies or sensitivities and may decline an activity involving a material that concerns me. 5. Pottery Wheels, Tools & Equipment I understand that pottery wheels, tools, bats, ware boards, sinks, shelving, and other equipment can cause injury if used improperly. I will secure loose clothing, jewelry, and hair as appropriate, keep my workspace reasonably clear, and stop using equipment if it appears damaged or unsafe. 6. Kilns & Hot Equipment I understand that kilns and recently fired pottery may reach temperatures capable of causing severe burns or fire. I will not operate, load, unload, open, touch, or access a kiln unless Clay Rose Pottery has specifically authorized me to do so. 7. Firing & Finished Work I understand that ceramics are unpredictable. Pieces may crack, warp, break, explode, stick to kiln shelves, experience glaze defects or color variation, or otherwise be damaged during drying, handling, glazing, firing, or storage. Clay Rose Pottery does not guarantee the successful outcome of any piece or firing. 8. Personal Property I am responsible for personal belongings, tools, artwork, and materials brought into the studio and understand that a shared studio carries some risk of accidental loss, damage, or breakage. 9. Health, Ability & Voluntary Participation I am responsible for deciding whether I can participate safely. I will tell staff if I need an accommodation, feel unsafe, become ill or injured, or need to stop. Participation is voluntary and I may stop at any time. 10. Assumption of Risk I knowingly and voluntarily assume the inherent risks associated with the activities I choose to participate in at Clay Rose Pottery, including risks that may result in bodily injury or property damage. 11. Release of Ordinary Negligence To the fullest extent permitted by Oregon law, I voluntarily release Clay Rose Pottery and its owners, instructors, employees, agents, and authorized representatives from claims for bodily injury or property damage resulting from ordinary negligence arising out of the covered activities and disclosed risks. This release is not intended to waive any claim that Oregon law does not permit to be released. 12. Emergency Assistance If I become injured or ill and am unable to communicate, I authorize Clay Rose Pottery staff to contact emergency services and my emergency contact as reasonably appropriate. I understand that I am responsible for medical expenses incurred on my behalf to the extent permitted by law. 13. Conduct & Shared Studio Responsibility I agree to act with reasonable care toward other participants, their work, and studio property; clean my work area according to studio procedures; and refrain from behavior that creates an unreasonable safety risk. Clay Rose Pottery may suspend studio privileges or ask a participant to leave for unsafe conduct or repeated rule violations. 14. Understanding & Voluntary Agreement I have had the opportunity to read this document and ask questions before signing. I understand that this document affects legal rights. I am signing voluntarily and understand that studio rules and safety procedures remain applicable after this waiver is signed.
I agree to all of the above rules and regulations.
PARTICIPATION CERTIFICATION I certify that the information I have provided is accurate and that I have read and agree to this Class, Workshop, Membership & Studio Use Acknowledgment of Risk, Release & Waiver.
