Permanent Jewelry Waiver & Release

Thank you for choosing our permanent jewelry service! Please carefully read and sign this waiver before your appointment.

1. Acknowledgment of Procedure

  • I understand that permanent jewelry involves welding a custom-fit chain directly onto my wrist, ankle, neck, or finger without the use of a clasp.
  • I acknowledge that while the process is generally safe, there are inherent risks, including but not limited to skin irritation, allergic reactions, or other unforeseen issues.

2. Assumption of Risk

  • I voluntarily assume all risks associated with the permanent jewelry process.
  • I understand that the jewelry is intended to be worn continuously, and removal may require cutting the chain.

3. Aftercare & Responsibility

  • I understand it is my responsibility to care for my jewelry after application.
  • I acknowledge that damage, loss, or breakage is not the responsibility of the business once the jewelry is applied.
  • I understand that my jewelry has a 10lb pressure break point for my safety, and that putting pressure on my jewelry will result in it breaking as a precautionary measure.

4. Sales Policy

  • All sales are final.
  • No refunds or exchanges will be given once the jewelry has been welded.
  • If the jewelry needs to be removed for any reason, a new purchase will be required to replace it.

5. Release of Liability

  • I release and hold harmless AMI Vibes its owners, employees, and contractors from any liability, claims, or damages that may arise from receiving permanent jewelry.

6. Consent

  • By signing below, I confirm that I have read, understood, and agree to all terms in this waiver. I consent to receive permanent jewelry and accept full responsibility for the outcome.

7. Deposit & Payment Terms

  • I understand that my $10 deposit is to hold my appointment time and will be deducted from my total.
  • I understand that if I cannot make my appointment, my $10 deposit will not be reimbursed.
  • I understand that my final balance is due at the time of my appointment (less my deposit fee).
