**ACKNOWLEDGMENT OF RISK AND WAIVER OF LIABILITY** 

I, the undersigned, am the parent or legal guardian of the child named above (“Participant”) and acknowledge that participation in activities at Adventure Alley involves inherent risks, including but not limited to falls, collisions, and other physical activities that may result in injury, illness, or other harm. 

**I understand and agree to the following:** 

1. **Assumption of Risk**: I understand that participation in activities at Adventure Alley is voluntary and may involve risks, including the risk of serious injury or death. I voluntarily assume all risks associated with participation in such activities.

2. **Waiver of Liability**: I, on behalf of myself, the Participant, and any heirs or assigns, hereby release, waive, discharge, and hold harmless Adventure Alley, Allison Manley, its owners, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by the Participant while participating in activities at Adventure Alley.

3. **Medical Treatment**: I consent to the administration of first aid and other medical treatment in the event of an injury, illness, or accident involving the Participant while at Adventure Alley. I understand that I will be financially responsible for any medical treatment required.

4. **Supervision**: I acknowledge that I am responsible for supervising the Participant at all times while at Adventure Alley. I understand that failure to properly supervise may result in injury or harm to the Participant or others.

5. **Rules and Regulations**: I agree to comply with all rules and regulations established by Adventure Alley or Allison Manley. I understand that failure to follow these rules may result in the Participant’s removal from the facility without refund.

6. **Governing Law**: This waiver and release shall be governed by and construed in accordance with the laws of the State of California. 

**By signing below, I acknowledge that I have read and understood this waiver and release of liability, and I voluntarily agree to its terms.**
