3. Athletic Waiver and Medical AuthorizationAssumption of risk. I understand that soccer, training and related physical activities involve inherent and sometimes unknown risks, including collisions, falls, contact with balls, other participants, walls, goals, nets and equipment, and hazards in spectator and common areas. These risks may cause serious injury, permanent disability, property loss or death. I confirm that the person participating is medically able to participate, will use appropriate protective equipment and will obey safety instructions. I voluntarily accept the inherent risks of participation and attendance. Release of ordinary-negligence claims. To the fullest extent permitted by Utah law, I release and agree not to sue US Comp Soccer, LLC and its owners, employees, contractors, volunteers, coaches, event officials and representatives for claims arising from my participation in USCS activities, including claims caused by their ordinary negligence. This release does not cover gross negligence, intentional misconduct or rights that cannot lawfully be waived. A parent or guardian’s signature authorizes the minor’s participation and acknowledges the risks; it does not waive a minor’s claims where prohibited by law. Insurance and responsibility. Participants and their families must maintain their own health or accident insurance and are responsible for their medical expenses. Membership does not provide medical or accident insurance. To the extent permitted by law, I agree to indemnify and hold USCS harmless from third-party claims and reasonable legal expenses only to the extent caused by my negligent or intentional acts or omissions; this does not require indemnification for USCS’s own negligence or a prohibited waiver of a minor’s rights. Medical authorization. Report injuries or illness promptly to staff and call 911 for emergencies. I authorize qualified medical personnel to provide necessary emergency care for me or the participant I represent, and accept responsibility for resulting treatment and transportation costs. Do not assume medical personnel will be on site.
4. Communications and MediaProvide a valid adult email address and mobile number for registration, schedules, cancellations and other program notices. By registering, I agree to operational and marketing communications from USCS and SLIS by email, text and app notification, subject to applicable law and available opt-out options. Promotional messages are subject to legally required consent and opt-out rights. To the extent permitted by law, I consent to USCS’s use of my or the participant’s image, voice or likeness in event coverage and promotional materials without compensation. For a minor, consent must be provided by the parent or legally authorized guardian.
5. Team Registration, Payments and RefundsCoaches and team representatives must submit a roster identifying all players, coaches and trainers, including player names, birthdates and jersey numbers. They must verify individual waivers and payments, communicate these rules, and supervise their team and associated spectators. Representatives are responsible for the payment balances they agree to incur. Home teams must bring two game balls. Memberships and event registrations require advance payment using USCS-approved payment methods. Processing fees may apply and are refundable only if returned by the processor or required by law. A $25 fee applies to returned checks. Unpaid balances may result in suspension of participation and lawful collection costs; nothing in this policy limits lawful payment-dispute rights. Refunds and scheduling. Payments are final except as stated here or required by law. Scheduling preferences, changes in opponents, opponent forfeits and participant withdrawals do not entitle a team or participant to a refund. USCS may offer replacement games or credits for scheduling errors, no-shows or similar disruptions. If USCS cancels a paid activity or cannot provide the purchased league/tournament placement, it will refund the unused paid portion, subject to lawful processing-fee deductions, unless the purchaser agrees to rescheduling or a credit instead. For fire, flood, severe weather, facility closure or other interruptions beyond USCS’s reasonable control, USCS may offer rescheduling or a credit for unused paid services. If the purchased services cannot be provided and the purchaser does not agree to an alternative, USCS will refund the unused paid portion, subject to lawful processing-fee deductions and applicable law. Facility Closure and Events Beyond USCS’s Control. US Comp Soccer, LLC (USCS) rents facility space and does not own or control the facility’s continued operation. If an activity is canceled, interrupted, postponed or relocated because of a facility closure, eviction, landlord lockout, loss of facility access, utility interruption, government order, or other circumstance beyond USCS’s reasonable control and not caused or contributed to by USCS’s breach of its rental obligations, negligence or intentional misconduct, USCS will not be responsible, to the fullest extent permitted by law, for resulting incidental or consequential losses, including travel, lodging, lost income or other related expenses. USCS will provide notice as reasonably practicable and make reasonable efforts to arrange an alternative location or reschedule affected activities. Refunds or credits for unused paid services will be handled under the Refunds and Scheduling terms above and applicable law. This clause does not eliminate rights that cannot lawfully be waived.
6. Participant AcceptanceBy signing or electronically accepting this waiver, I confirm that I am at least 18 and legally authorized to sign for myself or the participant I represent; I have read and understand these terms; and I voluntarily agree to them, including the release of substantial legal rights to the extent permitted by law. Electronic acceptance is intended to have the same effect as a signature. If I do not accept, the participant may not participate in USCS activities. Utah law governs this waiver. If a provision is held unenforceable, the remaining provisions remain effective to the extent permitted by law. This participant waiver does not amend the USCS–SLIS rental agreement or impose obligations on SLIS.Copy and paste the body of your waiver here. |
