Liability Waiver and Release Form Entry

Please take the time to carefully read and understand this entire document. By signing this Waiver and Release, you are giving up significant legal rights.

1. Acknowledgement and Assumption of Risk

I understand that participating in this course involves inherently hazardous activities, including:

  • Performing diving tasks and breathing compressed air underwater or in a hyperbaric atmosphere.
  • Working in, on, or around open water, machinery, and other potentially hostile environments.

I fully understand that these activities involve risks that could expose me to property damage, personal injury, or death. I hereby personally and expressly assume all risks connected with this course—whether foreseen or unforeseen—for any harm, injury, or damage that may befall me while enrolled as a student.

2. Remote Dive Sites and Medical Limitations

I further understand that the diving required for training and certification may be conducted at remote sites that are significantly distant (in terms of time, geography, or both) from proper medical facilities.

Nonetheless, I agree to proceed with these instructional dives. I choose to proceed with the course despite the possible absence of a nearby recompression chamber or immediate medical care, and I assume all associated risks.

3. Release of Liability and Indemnification

I agree that the instructors, employees, officers, agents, directors, and representatives of DiveSafe International and/or Korol Marine Services Ltd may not be held liable in any way for any occurrence in connection with this diving course.

It is my express intention by this instrument to exempt, release, and hold harmless the aforementioned program, companies, and their respective personnel from any and all liability, claims, or demands for personal injury, property damage, or wrongful death, even if caused by negligence. This agreement is binding upon me, my family, heirs, estate, and assigns.

4. Legal Competence and Voluntary Execution

I state that I am of lawful age and legally competent to sign this affirmation and release. I understand that the terms herein are contractual and not a mere recital. I freely, voluntarily, and expressly choose to accept and assume these risks, understanding full well the potential dangers.
