Integrative Medicine Treatment Consent - Dr Marie Paek

About Dr Paek and Next Practice Care of GenBiome

Dr Paek uses an integrated approach in her evaluation and treatment of individuals with a wide variety of problems. Her areas of expertise include nutritional medicine, digestive disorders, microbiome management, chronic fatigue syndromes, chronic pain syndromes, holistic mental health, women’s health stress and trauma reduction, natural treatments for addictions, allergies, autoimmune disorders, metabolic disorders, ADHD and autism spectrum disorders, anxiety disorders and functional neurological conditions. Many of her approaches are also targeted to prevention of chronic disease, including metabolic conditions and cognitive decline.




About the intake process with Dr Paek

The therapeutic approach to all these different conditions is looking for the underlying causes and addressing them systematically. In general, the common causes of chronic or recurring inflammatory conditions include a number of environmental factors, nutrition and lifestyle. Environmental factors include toxic chemicals, heavy metals, airborne allergens, food allergies or intolerances, mould and mycotoxins, tick and vector borne infections, persistent viruses, mild traumatic head injuries, emotional and childhood trauma, electromagnetism and radio frequency stresses. Lifestyle factors include stress management issues, sleep habits, physical activity, physical or structural dysfunctions and habits around food choices, timing of food and other nutrition factors.

Dr Paek and her team will be asking you about all these life factors in understanding the causation of your condition. She can then advise the range of investigations and treatments that can be directed to these issues.

 

About the pathology tests and laboratories used

Evaluations will follow a thorough history covering the above. Dr Paek will order blood tests including some appropriate Medicare rebatable items as well as non-rebatable items which are not covered by Medicare. A list of commonly requested tests with their prices can be provided to you before you decide to take the tests. Cost quotes for the other less common tests would have to be provided by the lab before you give the samples. Please ask for this at the lab before you agree to have the samples taken.

The preferred lab used by Dr Paek is Sonic Health Care Pathology- Douglas Hanly Moir in NSW.

 

Functional lab tests will also often be requested, through providers such as RN Labs, SIBO test, Nutripath, Microba, and others. These Australian distributors will invoice you directly. All of these tests are not covered by Medicare. Some of the tests are processed in USA and some in Australia. These are NATA accredited labs in Australia or their country of origin (for example CLIA in USA). Some tests are available online direct to consumer. Patients need to be aware that many conventional medicine providers may not be familiar with these labs or these tests and may therefore question their validity. Please always discuss your questions and concerns with Dr Paek and her team. Occasionally Dr Paek may discuss some lab tests which are not accredited by NATA. The pros and cons of doing all these tests will be discussed with you before deciding the most appropriate route of testing.

 

About your possible treatments with Dr Paek

Nutritional and environmental medicine treatments almost always involve one or more of the following:

  • Modification of the diet
  • Lifestyle changes to support sleep and physical activity
  • Nutritional supplements to support biochemical systems, detoxification pathways and other neuro-immune or hormone pathways, not only to treat deficiencies. Supplements may be over the counter, practitioner prescribed only or compounded products. Occasionally nutrients can also be provided intravenously or by injection
  • Environmental assessments and modifications as necessary
  • Detoxification therapies to remove environmental toxin accumulation
  • Colonic hydrotherapy to address constipation
  • Medication as necessary including antibiotics and pharmaceutical medicines used with routine as well as off label indications and compounded hormones
  • Treatment may help individuals reduce or eliminate symptoms, reduce or eliminate unnecessary medication, may correct metabolic imbalances via nutrition, may improve cellular and organ function via detoxification and reduction of pathogenic overload, could improve important lifestyle factors which have been impacted, such as sleep and resilience to stress.
  • There are patients who seem to have addressed many of these nutritional and environmental factors but there may be previously undiagnosed brain-based issues which block the body’s metabolism and biological response to treatment. With these patients, Dr Paek will work with you to identify what these factors are. This often involves referrals to other functional neurology and brain-based therapy (such as neurofeedback) providers and practitioners of chiropractic neurology.
  • Patients are advised to have their own GP for their normal health screens such as PAP smears, mammograms, immunisations and for events of acute medical matters.

 

About your appointment structures with Dr Paek and her team

You will first be required to go through an onboarding session with one of our patient advocates prior to your initial consultation with Dr Paek. 

 

An Onboarding Session is part of your induction at the clinic and assists in integrating you as a patient of Next Practice - GenBiome. This is an integral part of a nutritional and functional medicine assessment, which takes the time to learn about your case history and understand your health journey. This allows us to support you more and allows us to help you find a deeper understanding of the root causes of your health concerns.


During an Onboarding Session with a Patient Advocate, they will help you to complete the detailed onboarding form in the clinic, discuss your personal health goals, show you around the clinic and explain the modalities available whilst also showing you what might be involved in your program. Your body measurements will be taken including pulse oximetry, capnography, blood pressure, heart rate, body mass indices and waist-hip measurements. The Patient Advocate will help you set up your own patient app and supply you with any relevant initial educational materials.


This Onboarding Session is an opportunity to get to know your Patient Advocate who will be there to support you on your health journey and future appointments, as well as supply us with any existing pathology tests, lists of current medications and/or supplements. It gives us the chance to answer any of your initial questions, ensure that our philosophy is right for you and helps us set you up for success on your health journey with Dr Paek and the team.


New patient consultations require 75 minutes. Dr Paek will have discussed your case and reviewed your reports, results and information that you have provided prior to the appointment.

After your first appointment, you will have a follow up visit(s) for 60 mins 3-6 weeks after your initial visit, after further lab tests are back. Thereafter, visits are based on need – anywhere from 4-12 weeks or more apart and can be for variable duration. Dr Paek may see the benefit of referring you to other providers for integrated care. These providers maybe practicing at GenBiome or elsewhere. With chronic or complex conditions, people often fare better with a team approach. Dr Paek is regularly discussing her shared care patients with her team during in-house meetings as well as with external providers.

 

 

Costs of appointment, investigations and treatment

 

 

All appointment costs with Dr Paek are privately billed with Medicare rebates applicable. Pathology and radiology referrals are a combination of private and Medicare rebated items. All treatments are privately billed.

 

Fees for all consultations are based on the time you spend with Dr Paek. This means the fee will be adjusted pro rate temporis. This includes time spent with the patient as well as time spent outside the doctor-patient interaction, such as time spent on reviewing detailed history, results, preparation of comprehensive treatment plans which incorporate supplements, medications, diet and lifestyle recommendations

 

Your consultation time is charged at her current consultation rate.

Due to the nature of Integrative Medicine more time is usually required to better understand you as an individual in a holistic framework.


Some patients prefer to spend the time they need on the same day and are happy to pay for this time. Please be aware that appointments in this scenario can end up being rather long and are less supported by Medicare. As a result, you then incur a significant out of pocket/gap payment.


Other patients prefer to precisely stick to their booked appointment and come back another day to completely address their health issues and wellbeing. This would reduce your out-of-pocket expense. In this case please be mindful of what exactly you wish to focus on in the specified time so that your booked time slot can be adhered to.


Treatment Plans 

The review of comprehensive test results and the preparation of the corresponding treatment plans, including dietary/lifestyle recommendations and nutraceutical supplements and medications that may be appropriate, will incur an extra private fee (the cost being dependent on time spent needed to review results and create the plan outside of consultation time). Please notify your practitioner if you do not want such a plan to be prepared in writing and the review to take place verbally during your consultation time.

Reissue of previously provided documents such as treatment plans and pathology request will incur a fee of $25.

 

 Insurer Claims and 3rd Party Billing:

The payment of accounts in full at the time of consultation by the patient is preferred: the process is for the patient to pay for the service with their health care provider and then seek reimbursement from their insurer/3rd party provider.

GenBiome is not equipped, from a system and administration staff point of view, to deal with insurance or third party claims other than Medicare and Private Health Insurances.

If upon request, we make an exception to send invoices to 3rd parties and seek payment post consultation, we cannot guarantee any billing errors, such as duplication, will not occur. We will not be in a position to issue an invoice in the name of a third party.


Telehealth Consultations:

I agree to provide my Visa or Mastercard details at least 2 business prior to my appointment. Payment details are stored securely by the clinic to streamline the billing process after the consultation.

Bill payment shall be processed immediately at the conclusion of my appointment and email notification of payment is received. 

Standard delivery of documentation is via the Next Practice App. Any requested variation to this will incur additional charges.

I understand that telehealth appointments are not available to me if I do not agree to the payment terms as outlined above.




Consent to Treatment:

I consent to undergoing consultation, examination, clinical tests the nature of which has been explained to me.

 

I understand that Dr Paek practices Integrative medicine, which may be considered unconventional by the Medical Board of Australia. I will be able to seek further questions and clarification.

 I acknowledge that I hereby have been advised to maintain a relationship with a local general practitioner for routine health screenings and acute medical attention when needed. I understand that Dr Paek may not be able to see me with short notice and that her focus is largely on addressing conditions which are more often chronic or complex.

I also consent to undergoing treatments including the administration of medicines, compounded medicines, vitamins and minerals, compounded nutrient prescriptions and herbs. I understand that all treatment protocols will be explained prior to commencement.

 

I understand that some medicines being administered at GenBiome are used as off license or off label indication (this is when a medicine is used for any purpose for which it is not licensed, or in a dosage, or form of delivery for which it is not licensed.)

 

I understand that all the vitamins and nutrients prescribed and manufactured in Australia are approved by the Therapeutics Goods of Australia and that products manufactured outside of Australia are not TGA approved.

 

I consent to GenBiome sending prescriptions, supplement orders and other treatment requests to pharmacies and online supplement services that will contact me to process my product orders.

 

If I have chosen to provide my email address to Next Practice GenBiome, I understand that Genbiome may use this method of communication to contact me.

I understand that email transmission cannot be guaranteed to be secure or error-free and therefore Next Practice does not accept liability for any errors or omissions in the contents of messages which arise as a result of the transmission.

 

I consent to information from Next Practice Genbiome being sent to my health care providers who have referred me to GenBiome or have shared care.

 

If I do not wish for my information to be sent from Next Practice Genbiome, I will make this known to Dr Paek and the Genbiome team.

 

If I need reschedule or cancel my appointment, I shall notify the clinic or cancel my appointment via the patient app at least two business days prior to the appointment. I acknowledge and accept that a non-attendance fee of 100% of the consultation fee will be charged to me if I do not attend or notify in the aforementioned time frame.
