Billings Public Library CO+LAB Check In and Release Agreement

As a CO+LAB Creator, I will:

  1. complete the mandatory safety training course before using equipment in the CO+LAB.

  2. follow all established safety protocols and instructions for each piece of equipment.

  3. ensure that the equipment is returned in the same good condition in which I received it.

  4. follow instructions from library staff at all times.

  5. respect shared workspaces and equipment, by cleaning up after myself, including removing all materials, tools, and debris from work areas when finished.

  6. be courteous and respectful towards other users, fostering a collaborative environment where everyone can create and learn.

Release

 

I acknowledge that the CO+LAB materials and equipment can be inherently dangerous. I acknowledge that there is a risk of injury to person and damage to property from CO+LAB materials and equipment even when being used properly. I certify that I am aware of these risks and that by my or my minor child’s borrowing or use of CO+LAB materials and equipment I assume responsibility for these risks. With this knowledge, and in consideration for the right to borrow and use CO+LAB materials and equipment from the Billings Public Library, I hereby release and discharge the Billings Public Library and its Trustees, employees, agents, and contractors (collectively, the “Library”), as well as the City of Billings, its elected officials, mayor, and council members, employees, agents, and contractors (collectively, the “City”) from any and all suits, claims, and demands of any kind resulting from personal injuries, property damage, or otherwise of every kind and character arising out of 1.) my or my minor child’s borrowing or use of CO+LAB materials and equipment and 2.) any instruction and information provided by the Library, regardless of the form in which it is provided, as to the borrowing or use of CO+LAB material and equipment.

Indemnification

 

I agree to indemnify and hold the Library and the City harmless from any loss, damages, or costs, including but not limited to reasonable attorney’s fees, which may arise from any claim, demand, or litigation resulting from personal injuries, property damage, and/or other damages sustained by any person or entity arising out of or related to my or my child’s use or borrowing of CO+LAB materials and equipment. I will obey the CO+LAB safety precautions and recommended age guidelines as listed in writing for each item and will ensure the same for my minor child who uses or borrows any CO+LAB materials and equipment.
