I consent to having my data collected to be used to provide Worksafe B.C. and Northern Health with my personal information as it relates only to my tattoo procedure. Information taken is kept for records for a mandated amount of time. I understand that artists and employees at Black Magic Tattoo may collect pictures, audio and video data for security and promotions and my likeness is captured upon entering the premises. I also am giving permission to the artist to use pictures and or video of my tattoos for promotion and marketing. I cannot hold any tattoo artist liable for any adverse effects from healing my tattoo. I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I have been advised of the facts and matters set forth below and I agree as follows.I also have advised my artist/tattooist of the following: - If I have diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS, or any other communicable disease, heart condition or take medicine which thins the blood.
- I am not pregnant or nursing.
- I am not under the influence of alcohol or drugs.
- I do not have medical or skin conditions such as but not limited to: acne, scarring (Keloid), Eczema, psoriasis, rash, infection, lesion, freckles, moles or sunburn in the area to be tattooed that may interfere with said tattoo.
- If I have any type of infection, rash, or lesion anywhere on my body.
I acknowledge it is not reasonably possible for the artists and employees of Black Magic Tattoo to determine whether I might have an allergic reaction to the pigments or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible. I acknowledge that infection is always possible as a result of the obtaining of a tattoo, especially if proper aftercare wasn’t followed as directed. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touch-up work needed, due to my own negligence or failure to follow such instructions, will be done at my own expense. I realize that variations in color and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin color is dark, the colors will not appear as bright as they do on light skin, or other skin types. I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo. I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo. To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my well-being as a direct or indirect result of my decision to have a tattoo. I acknowledge I am over the age of eighteen and that I have truthfully represented to my tattooist that the obtaining of a tattoo is by my choice alone. I consent to the application of the tattoo and to any actions or conduct of the representatives and employees of the tattoo shop reasonably necessary to perform the tattoo procedure. Black Magic Tattoo discourages the use of numbing creams, spray and agents to be used by the client or the service provider in the act of receiving or administering a tattoo. I hereby release any artists and employees working out of black magic tattoos of any damages, or injury. The studio is not responsible for reckless behavior, secondary infection or time lost due to injury. I have been made aware of the risks and acknowledge the possibility of infection. I will take full responsibility, and solemnly promise to take best possible care of my new healing tattoo. Black Magic Tattoo AFTERCARE Thanks for trusting us with your new tattoo! We want to have clear and consistent instructions to help you with healing!! Please do not ask friends or family how to heal your tattoo, they didn’t do the work for you, and we have broken it down into simple steps that make it very easy to follow. You will need: - Soap. Unscented is best.
- Paper towel. Please use a paper towel to pat dry your new tattoo.
- Unscented lotion like Aveeno, Curel, Glaxal Base, or Lubriderm.
Steps to follow: - Always follow any special instructions given to you by your artist.
- Wear the bandage overnight or the time directed by your artist as weeping will occur.
- Remove in the AM, Shower, and make your tattoo the last thing you wash. Don’t be afraid, lather your hands with soap and give the tattoo a thorough cleaning. It may be sore and sensitive to high heat so adjust accordingly.
- Pat the tattoo lightly with a paper towel to dry, then let air dry.
- Wash your new tattoo 2 or more times a day for the first 2 days, this helps reduce healing issues. Especially if you notice its weeping.
- Continue to wash daily morning and night the upcoming days. It's important your tattoo stays clean, dry and out of the sun.
- If your tattoo gets very tight feeling a small amount of lotion can be used, your skin should never look wet with cream. Please massage it until fully absorbed.
- Your tattoo will flake or scab lightly. Please do not pick or scratch your new tattoo, it will harm it. If your tattoo is itchy, it's normal, lightly smack the skin instead of scratching.
- Continue with the washing regimen until your tattoo feels like your smooth skin again.
Some Do’s and Don'ts! - Pets' dander and hair can play a role in healing in an adverse way, be sure to sleep in clean sheets.
- Do not suntan during your healing process. After healing it's not recommended to expose your tattoo to direct sun. After healing make sunscreen part of your routine, as UV light can have damaging effects on your tattoo.
- No pools, hot tubs, lakes or soaking in the shower or tub during healing. Get in, wash and get out.
- Use CREAM sparingly. Many healing problems are cream related. Light use and fully absorbed is the way to go.
- Sports. Please be aware that you only have one chance to heal your tattoo. Excess movement, especially in areas that flex, stretch or pull can be harmful and effect healing. Gym equipment such as exercise machines or hockey gear can harbor destructive bacteria.
Issues - Redness and being sore is the name of the game when healing. It should only last 3-4 days. Sometimes longer. RED, HOT SWOLLEN and PAINFUL should not be the norm past 4-5 days. Infections do occur sometimes.
- Heavy Scabbing can occur at times but the washing regimen should be adhered to. Please do not pick, or drown with cream.
- Little pimples around the outside of the tattoo are often an asymptomatic issue of too much cream. Please email your artist.
