RELEASE AND WAIVER OF LIABILITY (the "Release")

To: Hockey Hive Ltd. (referred to in this Release as "Hockey Hive") IN CONSIDERATION of Hockey Hive allowing the person who has signed below (referred to in this Release as "you") to use Hockey Hive's facilities located at #3, 1010 – 12th Ave, Coaldale, Alberta T0K 0T0 (referred to in this Release as the "Facilities") and participate in and/or observe events and activities within the Facilities (collectively referred to in this Release as the "Activities"):

1. You acknowledge and agree that participating in or observing the Activities may cause damage to your personal property, personal injury or illness to you and/or may result in your death;

2. You confirm that your participation in or observation of the Activities is voluntary;

3. You assume the full risk of being at the Facilities and your participation in or observation of the Activities;

4. You fully waive all rights to make or bring any claims, actions, suits or proceedings against Hockey Hive, whether direct or indirect; and whether using equipment and/or the Facilities or not, all Participants, on behalf of themselves, and their parents, spouses, children/wards, heirs, assigns, representatives, estates, successors, attorneys, insurers, and all other persons, firms, partnerships or corporations connected therewith (collectively referred to hereinafter as the "Releasing Parties"), forever, finally, fully, permanently and unconditionally waive, release, acquit and discharge Hockey Hive, and its present and former employees, owners, members, principals, directors, subsidiaries, affiliates, representatives, predecessors, successors, shareholders, partners, parents, officers, agents, assigns, servants, attorneys, insurers, suppliers, manufacturers, clients, customers, participants, and all other persons, firms, partnerships or corporations connected therewith (collectively referred to hereinafter as the "Released Parties"), to the fullest extent permitted by law, from any and all charges, claims, debts, disputes, demands, suits, causes of action, rights of action, dues, sums of money, accounts, liabilities, losses, expenses and damages, absolute or contingent, known or unknown, whether or not asserted, threatened, alleged or litigated, now existing or arising in the future, at law or equity, whether caused by the negligence of the Released Parties or otherwise, that arise out of or relate in any way to Participants' use of the equipment and any of the other amenities at the Facilities, and any claims for costs, expenses and attorneys' and expert fees associated therewith.

5. You grant to Hockey Hive an unrestricted, irrevocable, royalty free right and license to use, reuse, broadcast, exhibit and publish throughout the world, in perpetuity, in any and all publications of Hockey Hive in any and all media, whether known or unknown, and in all forms, any and all of the digital images of you within the Facilities (the "Images"), or any part of the Images, without compensation to you whatsoever. You waive any and all moral rights or equivalent rights, provided under the Copyright Act (Canada), or otherwise, that you may have in the Images or in any part of the Images.

6. You release Hockey Hive from all claims, actions, suits, proceedings, liabilities, losses, damages, costs and expenses (collectively, "Claims") that relate in any way to your attending at the Facilities and/or your participation in or observation of the Activities, including, without limitation, any loss or damage to any of your personal property, and/or the Images;

7. You agree to indemnify and hold Hockey Hive harmless from and against any and all Claims that Hockey Hive may, directly or indirectly, suffer or incur in connection with or related to your use of the Facilities and/or your participation in or observation of the Activities and/or the Images;

8. This Release and Waiver of Liability is valid for a period of at least 2 years from the date of signing thereof;

9. For your safety, a CSA-approved helmet is mandatory while participating in activities at Hockey Hive. Failure to wear a helmet is at your own discretion and risk. By choosing not to wear a helmet, you acknowledge and accept full responsibility for any injuries or damages that may occur as a result.

10. Any misuse of equipment then its intended or instructed via verbal or written instructions use, That results in any damages to equipment is said person in this agreement responsibility of monetary replacement or fixing of said damaged equipment/facility.

11. By signing below, you agree that you fully understand and agree to the terms of this Release and that you are signing this Release voluntarily.
