To ensure the safety and protection of all our guests, we kindly ask that every RV owner provides a current copy of their RV insurance declaration page. This helps us maintain a secure, worry-free environment for all the families enjoying their stay.

By signing and submitting this form, you acknowledge the following:

1. Proof of Insurance
I am submitting a copy of my RV insurance declaration page, which includes the RV's make, model, VIN number, coverage dates, and liability coverage.

2. Accuracy
I confirm that the information provided is accurate and up to date to the best of my knowledge.

3. Responsibility
I understand that I am fully responsible for maintaining current insurance on my RV during my stay at Sleepy Hollow Lake Campground. I also acknowledge that failure to maintain coverage or defaulting in coverage may result in being asked to vacate the premises.

4. Indemnification
I release and hold harmless Sleepy Hollow Lake Campground, its staff, and ownership from any liability related to damages, accidents, or incidents involving my RV, to the extent permitted by law.

5. Compliance
I agree to follow all campground rules and local/state laws regarding RV insurance and registration.
