By Signing this Authorization Form, I agree that Silver Mountain may run my Credit / Debit card on the following dates for my season pass purchases. I also acknowledge that if my payment declines Silver Mountain will attempt to contact me via phone and/ or email. Failure to respond to Silver Mountain within five (5) days will result in a $25 late fee being assessed.

I also acknowledge that there is a one-time $25 payment plan fee, $60 for a family pass. Fees are assessed at the time of purchase. If I choose to cancel my purchase or stop making payments the payments and fees are non-refundable.
