Media Release

This Media Release ("Release") is effective as of the signing date ("Effective Date"), by and between the participant ("Releasor") and Cordova Fisher Design LLC (CFD) ("Releasee"), of 2828 North Central Avenue Suite 1100A, Phoenix, Arizona 85004.

I hereby irrevocably authorize CFD, and its agents, licensees, assignees, affiliates, and legal representatives, to edit, alter, copy, exhibit, publish, obtain copyright, or distribute any photographs, other images, video, or audio recordings, likenesses or other media featuring me ("Media"), captured or recorded for the purposes of publicizing, promoting, or advertising, or for any other related, lawful purpose. I acknowledge that since my participation with CFD is voluntary, no financial compensation will be received. Additionally, I waive any right to royalties or other compensation arising out of, or related to, the use of the media.

I understand that my image, voice, or the advertising copy or printed matter that may be used in connection with the use and/or publication of the Media may be edited, copied, exhibited, published, or distributed, and I waive the right to inspect or approve the finished product, including any written, printed, or digital materials, wherein my image, voice, or likeness appears.

I understand and agree that any photograph using my likeness will become the property of CFD and will not be returned.

This authorization of Release shall continue in perpetuity.

I further consent to the use of my name, or any fictitious name that may be chosen, in connection with the aforementioned Media.

I represent and warrant that, as of the date of this Release, (i) I have not granted any exclusive rights or licenses to the Media to any third party, and (ii) no other party's consent or authorization is required for the permission I am granting to the Releasee under this Release.

I hereby hold harmless, release, and forever discharge CFD (and all persons acting under its permission or authority) from any and all claims, demands, and causes of action that I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have arising out of or relating to this Release or the use of the Media. This includes, without limitation, any claims for libel, slander, invasion of privacy, infringement of copyright, right of publicity, or any other claim related to my likeness, including any claims related to blurring, distortion, alteration, optical illusion, digital alteration, use in composite form (whether intentional or otherwise), or the use of a fictitious name that may occur or be produced in the processing or publication of the Media.

Severability. If any provision of this Release shall be held to be invalid, illegal, or unenforceable for any reason, the remaining provisions shall continue to be valid and enforceable. If a court finds that any provision of this Release is invalid, illegal, or unenforceable, but that by limiting such provision, it will become valid, legal, and enforceable, then such provision shall be deemed to be written, construed, and enforced as so limited.

Entire Agreement. This Release contains the entire agreement of the parties with respect to the subject matter contained herein. No other promises, warranties, representations, agreements, or understandings, whether oral or written, exist concerning this subject matter. This Release supersedes any previous or simultaneous oral or written promises, warranties, representations, agreements, or conditions between the parties.

Governing Law. This Release shall be governed by the laws of Arizona.

I am at least 18 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand its contents, meaning, and legal effect, and I voluntarily agree to the terms of the Release.
