LAZER DUNGEON, LLC

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK: 

This form must be signed by the patron or, if the patron is a dependent, then by the parent or guardian before anyone can participate in any activities at Lazer Dungeon LLC.

I/We, the undersigned patron (or parents/guardians of a patron if they are under 18), have reviewed, understand, and agree to the information as follows:

I/We, the undersigned, recognize risks, including those of injury and death, in all activities initiated and carried out at Lazer Dungeon, LLC. I have read this form carefully and know that by signing this waiver, I freely volunteer to assume those risks on my own and/or for the dependents in my care.I will expressly assume the risk and legal liability and waive and release all claims for injuries, damages, or loss that I and/or the dependents in my care might sustain as a result of participating in any and all activities connected with and associated with Lazer Dungeon, LLC. 

I further agree to waive, fully release, and relinquish all claims I and/or the dependents in my care may have (or accrue to me and/or the dependents in my care) as a result of being on the premises and participating in activities at Lazer Dungeon, LLC., including its officials, agents, volunteers and employees (hereinafter collectively referred as "employees").

I have carefully read and understand the WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK agreement and fully understand its content. I am aware this contract releases Lazer Dungeon, LLC, and its employees from liability, and I sign this release of liability of my own free will. I DO UNDERSTAND THAT THIS LIABILITY WAIVER RELEASE IS EFFECTIVE FROM THE DATE SIGNED, UNTIL THE END OF THAT YEAR, AND MUST BE RENEWED IMMEDIATELY, THE FOLLOWING YEARS THEREAFTER, AT THE START OF A NEW YEAR.
