 Enrolment/Registration Agreement:

All participants and students MUST complete this form prior to participating in any classes with Lookout Dance Company.

 

If the participant is under 18, a parent or guardian 18+ must also sign this form.

 

Admission to class and/or rehearsal WILL NOT be granted if this form is not received prior to class or is not properly signed.

 

Deposit:

 

I understand that a deposit at the Pay-Per-Class rate for the class the dancer registers for must be made at the time of registration. I understand that the deposit will be used towards the dancer’s first class they attend. I acknowledge that this deposit is not refundable under any circumstances. I acknowledge that I have read Lookout Dance Company’s Registration Policy and agree to the terms outlined.


 

Class Payment:

 

I understand that the cost/fees for each class are subject to change at the discretion of the owner/director of Lookout Dance Company. I understand and agree that I will arrange payment prior to the commencement of class either at the Pay-Per-Class rate, Class Pass/Loyalty Program, and/or Registration Fee Package. I understand and acknowledge that I will provide payment in advance up to 15-minutes to the start of class. I understand that I will not be able to enter class if payment is not received 15-minutes prior to class, unless it was discussed and acknowledged by the owner/director of Lookout Dance Company. I acknowledge that I have read Lookout Dance Company’s Payment/Fee Policy and agree to the terms outlined.


 

 

Refunds:

 

I understand and acknowledge that all class payments and deposits sent to Lookout Dance Company from myself via e-transfer, cash, or other methods are not refundable due to absences, misses, registration cancellations by Lookout Dance Company or myself, etc. Class payments will only be refundable due to class cancellations notified by Lookout Dance Company via their Facebook page, Facebook group, and/or Instagram page. I acknowledge that I have read Lookout Dance Company’s Payment Policy and agree to the terms outlined.

 


 

Registration Cancellation:

 

I understand that I must notify Lookout Dance Company via email/text/call to cancel my registration as soon as possible. I understand that by cancelling my registration, I will be removed from the registration list. I understand that if I decided to rejoin Lookout Dance Company and there is no spot immediately available, I will be put on a waitlist. I also understand that if I miss 3 scheduled classes consecutively without notice/reason given to Lookout Dance company, my registration will be cancelled. I understand I will be notified via call/text/email that my registration is cancelled due to 3 consecutive misses and will have 24 hours to respond prior to cancellation. I acknowledge I have read Lookout Dance Company’s Registration Policy and agree to the terms outlined.

 


 

 

Waitlist/Interest List:

 

I understand that if there are no spots available at Lookout Dance Company, I will be put on a waitlist/interest list and notified when a spot opens. I understand that I will not need to pay the deposit at the time of signing up for the waitlist. I understand that once I am notified via call/text about an available spot, I will need to provide a deposit within 24 hours of notification, otherwise, I will lose my spot in the company. I acknowledge I have read Lookout Dance Company’s Registration Policy and agree to the terms outlined.


 

            

Dress Code:

I understand that dance requires proper attire to ensure the safety of myself and others during class. I understand and agree to come to class dressed appropriately as outlined in the Dress Code Policy. I am aware these are additional costs and not included in class payment. I am aware that I will not be permitted to attend class if I do not arrive in proper attire. I acknowledge I have read Lookout Dance Company’s Dress Code Policy and agree to the terms outlined.


 

 

Schedule Changes:

 

I understand that classes could be cancelled, or the day/time could be changed at the discretion of the owner/ Lookout Dance Company. I understand that I will be given notice via Lookout Dance Company’s Facebook page, Facebook group, and/or Instagram page about class cancellations. I acknowledge I have read Lookout Dance Company’s Facility Policy and agree to the terms outlined.


 

 

Holidays/Closures:

 

I understand that Lookout Dance Company generally follows the school year and Sioux Lookout Recreation Centre’s schedule including scheduled holidays. I understand that classes are not held on holidays and/or facility closures due to maintenance/power outages/ etc. I understand holidays/closures do not count as class cancellations as they are scheduled downtimes.

 


            

Facility:

 

I acknowledge and agree to respect and care for the rental facility and the equipment/property of Lookout Dance Company. This includes utilizing equipment appropriately, keeping personal belongings tidy, and cleaning up after oneself. I understand that parents/guardians, dancers, and/or guests that cause significant damage, will be held financially responsible for repair. I understand that only closed-lid water containers are allowed in the studio space and that outdoor shoes are not permitted on the dance floor. I acknowledge and agree to follow the nut-free policy for the safety of others/dancers. I acknowledge I have read Lookout Dance Company’s Facility Policy and agree to the terms outlined.


 

Photography/Videography:

 

I agree to not take photos/videos during class time due to privacy concerns acknowledged by our members in the Liability Release/Waiver Form. I understand that Lookout Dance Company staff may take videos/photos of the class for promotional/advertising/marketing purposes for use on social media and our websites. These photos may also be used on flyers, posters, brochures, and business cards with/without a name attached. I agree to only take pictures/videos of myself and/or my child before and after class. I understand that no other child should be in the pictures/videos unless acknowledged by their parent/guardian. I understand that I can take pictures/videos of myself and/or my child with the instructor and/or Lookout Dance Company staff. I understand that photography/videography by dancers and/or their families during class may be acceptable at the discretion of Lookout Dance Company for the purposes of instruction and/or learning. I also understand that dancers and/or their families may be able to take photos/videos during class for personal use but not for social media/promotional purposes at the discretion of Lookout Dance Company.


            

Dancer Etiquette:

 

I agree to arrive no earlier than 5-10minutes before my scheduled class. I understand I will not be allowed into class if I am greater than 5-10 minutes late to class unless otherwise discussed with and acknowledged by the owner/director of Lookout Dance Company. I understand that I am to wait quietly while the teacher is giving instructions and/or while other dancers have their turn. I agree to not lean or hang on the barres/equipment provided by Lookout Dance Company and/or the rental facility. I understand that my teacher/instructor may use “hands on” instructions as well as verbal instructions to correct my technique. These corrections may require a teacher/instructor to touch my arms, legs, feet, hips, abdomen, back and head during class to ensure proper alignment and to help me avoid injury. I acknowledge I have read Lookout Dance Company’s Class Etiquette Policy and agree to the terms outlined.



Parent/Guardian Etiquette:

 

I agree to wait with my dancer until the class has commenced and promptly pick them up at the end of class. I agree to call/text/Facebook message/email to report absences or late arrivals. I understand to stay up to date on class cancellations, meetings, events, workshops, closures, etc.

Inclusivity:

 

I agree to treat all teachers/instructors, parents/guardians, rental facility staff, visitors, and dancers with respect and commit to promoting an inclusive and welcoming environment for all. I understand that Lookout Dance Company follows an inclusive, non-discriminatory and antibullying approach in their classes. I understand that Lookout Dance Company has the right to remove anyone who is rude, offensive, and/or physically/verbally inappropriate from the facility and/or class. I acknowledge I have read Lookout Dance Company’s Inclusive Environment Policy and agree to the terms outlined.
