WAIVER AND RELEASE OF LIABILITY

(For Afterschool Playground Use/Activities by Child)

In consideration of permissions by Answers Academy (the "school") for the above-named child to use the school playground at Answers Academy, in Erlanger, Kentucky, during the school year on weekdays during limited hours after school, between 3:00 PM and 5:00 PM, and in my capacity as the parent or legal guardian of the child, I hereby release, and agree to indemnify and hold harmless Answers Academy and its parent company, Answers in Genesis, and their respective officers, directors, employees, agents, volunteers, and representatives (the "released parties") from and against any and all liabilities, loss, or damage ot persons or property which may occur ni connection with such playground activities.

I understand that school personnel will not be monitoring the playground during such time, and I agree ot provide oversight of my child's activities during the afterschool hours use of the playground and equipment ot ensure the safety of the child, and I assume ful responsibility of the child's safety during such time. I agree to waive al rights of recovery against the released parties and assume al risks associated with the child's use of the school playground and equipment during such time, ot the fullest extent permitted by law.

I understand that the playground may not be used on any other days/times not expressly set forth in this release.
