PLEASE READ EACH SECTION CAREFULLY. Once you have signed, you will be asked for your contact information and more about yourself. I do hereby agree to volunteer my services to Battleship Cove. I understand and agree that as a volunteer, I am not entitled to, nor do I expect to receive, any compensation for my services or a guarantee of any future employment. All reimbursement for personal expenses must be agreed upon in advance, in writing, with my volunteer program supervisor. I understand volunteers are not authorized to enter into contracts or other binding agreements on behalf of Battleship Cove. I understand that safety is of the utmost importance when performing my duties. I will adhere to all safety guidelines and directives from my supervisor or other members of staff. Failure to do so will constitute grounds for immediate dismissal. I understand that I am required to attend an orientation and training session. I understand that I will need to undergo a criminal history check (CORI) Code of Conduct - I understand and agree to the following while volunteering at Battleship Cove:
- I will not enter restricted areas without permission from a supervisor or staff member.
- I will abide by all safety guidelines and directives from my supervisor.
- As a representative of the museum, I will report for my duties clean, neat, and appropriately attired.
- I will wear my ID badge at all times.
- I will treat others with respect, patience, courtesy, and tact.
- I will make every effort to be knowledgeable about the subjects entrusted to me.
When children are present: Volunteers should avoid being alone with a child. Under no circumstances shall a volunteer release a child to anyone other than a parent or authorized guardian. I understand that the following are strictly prohibited and will report all violations to a supervisor or staff member immediately: - Deliberate or neglectful waste.
- Damage to or theft of museum, personal, or client property.
- Profanity or any kind of harassment.
- Using or possessing illegal drugs on the museum property or having alcohol or drugs in one’s bodily system while carrying out volunteer duties.
- Possession of weapons and/or live ammunition. Note: under certain circumstances historic weapons or replicas and blanks are permitted - permission must be obtained in writing BEFORE entering the property.
I have read the above and understand that any violation of this Code of Conduct may result in disciplinary action including, but not limited to dismissal. I understand that either party may terminate this Agreement and with it my services at any time, with or without notice and with or without cause. No penalty, liability, or compensation will result from such termination. Photo Release I authorize Battleship Cove to publish photographs of me, and my name, in print and online. Since my participation is voluntary, I will receive no compensation for said publication, and release Battleship Cove from all liability. I further agree that my participation in any publication or website produced by Battleship Cove confers upon me no rights of ownership.
Notice of Asbestos, Lead, and PCBs/ Release of Liability Battleship Cove makes every effort to create a safe working environment and to eliminate the risk of involuntary exposure to hazardous materials. However, it must be understood that materials containing asbestos, lead, and PCBs were commonly used in naval vessels and such materials may still be present. Breathing asbestos dust or exposure to lead or PCBs can cause serious bodily harm and/ or death. All crewmembers are advised that: - Some types of shipboard insulation materials contain asbestos fibers. Do not disturb insulation and avoid creating dust.
- Some shipboard coatings contain lead. Do not sand, grind, chip, or otherwise disturb a painted or coated surface without proper personal protective equipment and dust abatement measures in place.
- Polychlorinated Biphenyls (PCBs) are manmade chemicals that were used in multiple industrial applications before their manufacture was banned in 1979. These include:
• transformers and capacitors • electrical equipment including voltage regulators, switches, re-closers, bushings, and electromagnets • oil used in motors and hydraulic devices • fluorescent light ballasts • cable insulation • thermal insulation material including fiberglass, felt, foam, and cork • adhesives and tapes • oil-based paint • caulking • plastics • carbonless copy paper • floor finish - Use care when working on or handling old mechanical equipment or light fixtures.
- Wash hands thoroughly before eating.
- Shower and change clothing immediately after completing work.
- Launder all work clothing thoroughly and use a separate container for soiled work clothing.
The volunteer does hereby freely for the volunteer, the volunteer’s heirs, administrators and assigns, release, remise, and discharge Battleship Cove and the United States, their agents, their servants, and their employees from any claims, demands, actions, and services of action of any sort for injury sustained by the volunteer-related to the individual presence and activities of the volunteer on the property, including any injury that is caused or worsened in whole or in part associated with exposure to asbestos, lead, or PCBs. The volunteer further acknowledges that the volunteer assumes sole and complete responsibility for all of the risks associated with such presence, activities, and exposure. The volunteer hereby agrees to indemnify and hold harmless Battleship Cove and the United States, their agents, their servants, and their employees against all loss, damage, or claim of liability whatsoever, due to the personal injury or death or damage to property of others caused either directly or indirectly by the exercise by the volunteer of the privilege granted by this permit. I have read, understand, and agree to all the above-listed notices, releases, and expectations.
