FREEDOG BRISTOL CONSENT & ACCEPTANCE OF RISK FORM

IN CONSIDERATION OF being permitted to use trampolines, soft play and related facilities within Freedog Bristol. The undersigned acknowledges and agrees to the following conditions;

ACCEPTANCE OF RISK

Activities in Freedog Bristol are physically demanding, and the risk of injury is significant. The chances of minor injury (bruises, bumps, friction burns and minor fractures) have been seen as possible outcomes to much of the same extent as normal active play. However, there is potential for serious bodily injury, permanent disability, paralysis and loss of life. Participants (or parents / appointed guardians) must accept that such risks cannot be entirely eliminated while maintaining the adventurous activities in Freedog Bristol.

CONDITIONS OF USE

Our Duty Of Care

The rules of Freedog Bristol set out below are not intended to limit the enjoyment of the facilities. They are part of the duty of care that we, as operators, owe to you, the customer, by law. As such, they are not negotiable, and if you are not prepared to abide by them, the staff may ask you to leave.

Your Duty Of Care

You also have a duty of care to act responsibly towards other users of the centre, our staff and facilities. Statements of ‘Good Practice’ are posted around the centre adjacent to the relevant facilities.

Behaviour

Participants (or parents / appointed guardians) must understand that in the interests of safety, participants must follow the rules of Freedog Bristol as shown in this document, on signage in the centre, in safety briefings and in instructions given by staff members at all times. If your session is stopped due to the behaviour of the participant, no refunds will be given.

Our staff are on-site to help provide a safe environment. We do not tolerate abusive or threatening behaviour towards our staff.

Photos

Freedog Bristol may use any photos taken of you (or the children you are accompanying) in the centre, on its website, print media and social media. You must inform the Duty Manager whenever you enter the building if you do not wish your image to be used.

Alcohol and Drugs

Being under the influence of alcohol and/or drugs when using Freedog Bristol’s facilities is strictly forbidden. The acceptable level of alcohol or non-prescription drugs in any participant's system is zero. If the staff suspect or know that a participant has had an alcoholic drink or has taken non-prescription drugs, they may be refused participation or entry.

GENERAL RULES

  • Participants must abide by all rules and instructions regarding Freedog Bristol’s activities; therefore, participants must;
  • Complete an acceptance of risk form before participating in any activity or supervising a participant in the soft play area.
  • Report to reception upon arrival.
  • Attend a safety briefing for their chosen activity upon every visit.
  • Familiarise themselves with all safety-related signage.
  • Seek clarification from staff if they do not understand any of the rules or instructions.
  • Running is not permitted anywhere in the centre.
  • Participants must exercise care, common sense and self-preservation at all times.
  • For the safety of all participants and others, if our staff think a participant cannot safely complete an activity, they will communicate this to the participant(s), and the participant must then stop.
  • Participants must report problems with the equipment, facilities and other participants’ behaviour to staff immediately.
  • Freedog Bristol reserves the right to refuse entry.
  • Participants must be aware of the other customers around them and how their actions will affect them.
  • Customers must not distract participants while they are using the equipment.
  • To the best of their knowledge, participants should not suffer from any medical condition or other conditions that may prevent them from participating in any of the activities or make it more likely that they will be involved in an incident that could result in injury to themselves or others.
  • Freedog Bristol’s staff are on hand to attend to any first aid requirements. If a participant has an injury, it is their responsibility to inform a member of staff.
  • Due to our manufacturer’s guidelines regarding trampolines, a maximum weight limit of 140kg is enforced.
  • Children under the age of twelve must always have an accompanying adult within the building.
  • Appropriate attire to cover arms and legs is advised to reduce the risk of abrasions and friction burns on the equipment.

Use of Freedog Bristol’s Trampolines, Foam Pit, Interactive Games and Ninja Course:

  • Freedog branded socks must be worn at all times.
  • We cannot guarantee the quality of other sock manufacturers/providers.
  • For the safety of all participants, each session offering has its own age and height limits. For this information, participants can refer to the most up-to-date version of the website.
  • The person making the booking is responsible for choosing an appropriate session for the participants' age and height. No refunds are offered for inappropriately booked sessions, and proof of age may be required.
  • All participants must attend a safety briefing before entering the activity area. Participants must watch the safety video and be satisfied that they fully understand all instructions.
  • All participants must understand and know their own limits. Participants must not try anything they feel may be beyond their capabilities or present a danger to themselves or others.
  • Each apparatus in the activity area has signposted rules. It is the participants' and accompanying adults' responsibility to read and follow these rules and instructions given by staff during the briefing and session.
  • Participants under 5 are not permitted to use the Ninja course activity.
  • While using the Ninja course, participants must not try any obstacle they feel they cannot complete safely.
  • Whilst using the Ninja course and interactive games, participants must not climb on the apparatus beyond its intended use.
  • Usage of our Ninja course is one participant per lane.

Use of Freedog Bristol’s Toddler Sessions:

  • Freedog branded socks must be worn at all times.
  • We cannot guarantee the quality of other sock manufacturers/providers.
  • Toddler-specific sessions are for children aged 5 and under AND under the height of 110cm.
  • Participants must receive a safety briefing every time before taking part in the session.
  • Accompanying adults must directly supervise their toddlers at all times.
  • Accompanying adults may only supervise up to two children.
  • Participants must respect and not damage the toys provided.
  • Accompanying adults agree that their child can fully participate in the session and has no injuries, illnesses or medical conditions that may be adversely affected by participation.
  • Accompanying adults must keep their bouncing to a minimum and be aware of other participants.
  • Due to height restrictions, the Ninja course will not be available during this session.

Use of Freedog Bristol’s Soft Play:

  • Soft play is unsupervised by staff, and the accompanying adult/s are responsible for ensuring their child's safety.
  • Accompanying adults must directly supervise their children at all times.
  • Accompanying adults may only supervise up to two children.
  • For the safety of all participants, each soft play area has its own age and height limits.
  • Puppy Pen: This is for children 0-2 years of age and must be directly supervised by an adult.
  • Main area: This area is for children 2-7 years of age and must be under the height of 135cm.
  • Each area of soft play will have its own set of rules that accompanying adults should familiarise themselves with and encourage children to follow.
  • Food and drink must not be taken into the soft play without exception.
  • Socks must be worn in this area. However, they do not have to be Freedog branded socks.
  • Participants must respect and not damage the toys provided.

Legal Declaration:

  • I am a competent adult (18 years or older) participating in Freedog Bristol’s activities, or I am the parent, legal guardian, or appointed guardian (as defined below) of a child (under 18) participating in Freedog Bristol’s activities.
  • If this agreement is signed by me on behalf of a child, I confirm that the child’s full name and date of birth is correct, and that either;
  • I am the parent / legal guardian of that child OR
  • When I am not the parent / legal guardian of that child, the child’s parent or legal guardian has permitted me to assume that responsibility for that child and for me to have the right to sign this agreement on their behalf (as the appointed guardian)
  • I (/the child) understand that activities in Freedog Bristol are physically demanding, and the risk of injury is significant. The chances of minor injury (bruises, bumps, friction burns and minor fractures) have to be seen as possible outcomes to much the same extent as normal active play, but I (/the child) also understand that there is potential for serious bodily injury, permanent disability, paralysis and loss of life. I acknowledge that such risks cannot be entirely eliminated without jeopardising the quality of Freedog Bristol’s activities and that while following the rules and instructions given may reduce the risks, the risk of injury and loss of life does still exist.
  • To reduce the inherent risks of the activities, I (/the child) must follow the rules of Freedog Bristol as shown in this document, on signage in the centre, in safety briefings (verbal and video) and instructions given by Freedog Bristol staff at all times.
  • I (/the child) must disclose any condition that may affect my (/the child) participation in the activities at Freedog Bristol. I (/the child) understand that Freedog Bristol staff are not medical professionals, and they cannot assess if any condition might make the activity more dangerous or inappropriate. It is my responsibility to seek medical advice before I (/the child) take part in the activities at Freedog Bristol.
  • Freedog Bristol provides first aid trained staff and equipment in case of injury, accidents or illnesses during participation. I consent to me (/the child) receiving treatment that may be deemed advisable whilst on Freedog Bristol’s premises.
  • Freedog Bristol will only be liable for loss or damage suffered as a result of your (/the child’s) participation if it is caused by Freedog Bristol’s failure to carry out its obligations or breaches of any duties imposed by law. Freedog Bristol will not be liable for loss of damages suffered if they occur due to (i) my (/the child’s) own fault, (ii) the provision of services or products by a third party, or (iii) events which Freedog Bristol could not have foreseen or prevented even if Freedog Bristol had taken all reasonable care.
  • My (/the child’s) involvement in the activities in Freedog Bristol is voluntary, and I am (/the child is) acting under my (/their) own free will.
  • I am solely responsible for my own (/the child’s) conduct and actions while participating in Freedog Bristol’s activities.
  • If any part of this agreement is invalid or ineffective, the remainder will continue in full force and effect.
  • The laws of England and Wales govern this agreement.


I AM AWARE THIS IS A CONTRACT, AND I SIGN IT OF MY OWN FREE WILL.
