I, HEREBY AFFIRM THAT I HAVE BEEN ADVISED AND THOROUGHLY INFORMED OF THE INHERENT HAZARDS OF SNORKELING ACTIVITIES, INCLUDING, BUT NOT LIMITED TO THE INFORMATION WHICH I HAVE READ SET OUT BELOW.
