RELEASE AND WAIVER OF LIABILITY AGREEMENT 

I, a parent or guardian of a participant of Champ Camp, acknowledge that I have voluntarily applied for him or her to participate in Champ Camp.

 

I AM AWARE THAT CHAMP CAMP ACTIVITIES ARE POTENTIALLY DANGEROUS ACTIVITIES AND THAT MY CHILD COULD BE SERIOUSLY INJURED OR HARMED WHILE PARTICIPATING IN THEM. I AM VOLUNTARILY ALLOWING MY CHILD TO PARTICIPATE IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED, AND AGREE TO ASSUME ANY AND ALL RISKS, WHETHER THOSE RISKS ARE KNOWN OR UNKNOWN.

 

In consideration of my child participating in Champ Camp activities, I forever release Champ Camp LLC, any affiliated organization, and their respective directors, officers, employees, volunteers, agents, contractors, and representatives (collectively “Releasees”) from any and all actions, claims, or demands that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives now have, or may have in the future, for injury, death, or property damage, related to (i) my child’s participation in these activities, (ii) the negligence or other acts, whether directly connected to these activities or not, and however caused, by any Releasee, or (iii) the condition of the premises where these activities occur, whether or not my child is then participating in the activities. I also agree that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives will not make a claim against, sue, or attach the property of any Releasee in connection with any of the matters covered by the foregoing release.

 

I also grant all Releasees the permission to take photographs and/or videos of my child in connection with his or her participation in Champ Camp activities. I authorize the releases to copyright, use, and publish the photo and video content. I agree that the Releasees may use such photo and video content for any lawful purpose, including but not limited to such purposes as publicity, illustration, advertising, social media, and web content.


 

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. FURTHERMORE, I VERIFY THAT I TRUTHFULLY COMPLETED THE ENTIRE REGISTRATION FORM MYSELF AND HEREBY AGREE TO ITS CONTENT IN ITS ENTIRETY. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND CHAMP CAMP LLC, AND SIGN IT OF MY OWN FREE WILL.

I verify that the dangers of the activities and the significance of this Release and Waiver were explained to the Participant and that the Participant understood them.

 

By signing this agreement, I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Champ Camp LLC or participation in Champ Camp LLC programming (“Claims”). I consent to Champ Camp LLC providing first aid, including calling paramedics and I assume financially responsible for all costs related to aid. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Champ Camp LLC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Champ Camp LLC, its employees, agents, and representatives.
