Acknowledgement and Assumption of Risk

As the legal guardian/parent of or representative of myself, I hereby authorize Jordan Warriors LLc., with offices at 140 Epping Rd Exeter, NH 03833, or anyone authorized by Jordan Warriors LLC, to use and reproduce all pictures, videos, voice dialogue, or other audio of me or my likeness. I authorize Jordan Warriors LLC to use all data, including my name, hometown, social media handles/names, or any other information disclosed by me to Jordan Warriors LLC, and all other information regarding my participation in Jordan Warriors LLC events (collectively "materials") without any notice or compensation to me. The materials shall remain the property of Jordan Warriors LLC or its designees in perpetuity.

Such materials will be used in connection with the business of Jordan Warriors LLC or its designees in a manner authorized by Jordan Warriors LLC. The rights set forth in this waiver include but are not limited to, the right to use, copy and modify such materials in connection with websites, youtube, facebook, twitter, and Instagram accounts owned or operated by Jordan Warriors LLC or its designees. The materials may be exploited throughout the world without notice or compensation to me whatsoever.

I Agree


As the legal guardian/parent of or representative of myself, I acknowledge that participation in Jordan Warriors LLC events may be hazardous and I hereby assume all responsibility and risk for any accidents, sickness or other mishaps, including, but not limited to, death, serious bodily injury, permanent disability, that may result to me while competing and training with Jordan Warriors LLC. I represent that I (my child) is in good physical condition, and I am otherwise physically capable of participating in ninja training and Jordan Warriors LLC events that involve running, jumping,climbing and hanging for extended periods of time. I (my child) recognize my heart rate will be elevated at all times during training and events, and I (my child) do not suffer from any condition with may endanger my safety or the safety of others participating in the training and/or event. Participation in Jordan Warriors LLC facility activities entails known, anticipated and unanticipated risks that could result in physical and/ or emotional injury, paralysis, death or damage to yourself and/or to others. Risks may include but are not limited to, slipping and falling, landing improperly, collisions with fixed objects, moving objects and/or people with may result in strains, fractures, breaks, scrapes, bruises, dislocations and injuries to head, back and neck.

I Agree

I (my child) agree to abide by the facility rules and instructions and the directions of Jordan Warriors LLC employees and representatives, whereby I acknowledge that those rules, instructions and directions are intended to promote the safety of both myself and others; my failure or refusal to abide by those rules, instructions and directions can lead to the immediate revocation of my rights to use Jordan Warriors LLC and its facilities, without any right to refund of any payments made; and in the event of sickness, accident or injury, I (my child) will immediately report my injury to Jordan Warriors LLC ’s staff and under no circumstances will I leave the facility without doing so, I will cease all participation in the facility activities at that time of sickness, accident or injury, I authorize Jordan Warriors employees and representatives to obtain and secure, on my behalf, emergency medical treatment and transportation, when deemed appropriate by Jordan Warriors' employees and representatives and I agree assume, at my expense, all costs of emergency medical care and transportation.

I Agree


I (my child) voluntarily, of my own free will, and on behalf of my executors, past present and future heirs, assigns and personal and legal representatives do hereby waive, release, and forever discharge Jordan Warriors LLC and all of its officers, directors, employees, subcontractors, agents and representatives from and against any and all claims, debts, demands, rights, liabilities, causes of action, losses , damages, costs or expenses, including reasonable attorneys fee (collectively, claims) of whatsoever kind and nature arising, resulting from, or relating in any way to ninja warrior training or competition. I agree and covenant not to sue or bring any claims against Jordan Warriors LLC or any of its respective members with respect to any matters that result from or are related to ninja training or events associated with Jordan Warriors LLC. In the event that I or anyone else on my behalf institutes any such action, that claim shall be dismissed upon presentation of this waiver and release and I will reimburse Jordan Warriors LLC or its officers for any and all legal fees or expenses relating to defending such a claim and obtaining its dismissal.

I Agree


I have read this waiver and release and I fully understand all related terms, understand that I have given up substantial rights by signing this waiver and release, and represent that I am signing this waiver and release freely and voluntarily without any inducement. I also certify that I have appropriate insurance coverage for any and all possible injuries that may occur at or in connection with any Jordan Warriors LLC related events or training in which I (my child) am participating.
