Agreements: At Beehive, I agree to: 1. stay with and monitor my child(ren) at all times, to help them navigate social situations and attend to their emotional and behavioral well-being. 2. respect and welcome all families into our community. 3. show kindness and empathy to all visitors and staff. 4. help maintain the space by cleaning up after my child to the best of my ability. Beehive rules: 1. Beehive is a shoe-free facility. Please wear socks or play in bare-feet. If there are extenuating circumstances, our front desk staff can help you get shoe covers. 2. Outside food and drink is welcome at Beehive. Please keep all food items and eating in the classroom area if available. If not available, please do not allow children to walk around with food and drink but instead find a spot to sit and snack. 3. Please stay home if you or your child are feeling sick to protect our whole community. 4. Children are never permitted to throw objects over the slide area or bring pillows down the slide. It is the caregiver's job to ensure this rule is followed. 5. Parents must accompany their children to the bathroom at all times. Our bathroom is semi-shared.
6. Before play, please remove all hood and neck drawstrings from children’s clothing. 7. Beehive is not responsible for lost, stolen or damaged belongings. 8. And of course, all members of the community are encouraged to visit Beehive.
WAIVER I understand that the use of any child play facility inherently entails risk. Accordingly and in consideration of Beehive: A Community Playspace (hereinafter known as “Beehive”) granting access to myself and the children identified on the list below (the “children”) to Beehive’s child play facility and the equipment and other amenities and other ancillary items located in the facility (collectively, the “facility”), I expressly agree and contract, on behalf of myself and the children and each of our respective heirs, executors, personal representatives, administrators, successors and assigns, that neither Beehive nor its insurers, members, employees, officers, directors, or associates shall be liable for any injuries (including death), damages (both economic and non-economic), and/or losses of any type sustained by me or the children on or about the facility except to the extent that the same results from the negligence or intentional misconduct of Beehive. By signing this agreement, I hereby fully and forever release and discharge Beehive, its insurers, members, employees, officers, directors, and associates, from any and all claims, damages, demands, rights of action, causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out of the use of the facility except to the extent that the same results from the negligence or intentional misconduct of Beehive. COVID-19 SAFETY INFORMATION:
While participating in play, classes, events or any group or program held by Beehive: A Community Playspace, consistent with CDC guidelines, participants are encouraged to practice hand hygiene, “social distancing” and wear face coverings to reduce the risks of exposure to COVID-19. Because COVID-19 is extremely contagious and is spread mainly from person-to-person contact, Beehive: A Community Playspace has put in place preventative measures to reduce the spread of COVID-19. However, Beehive: A Community Playspace cannot guarantee that its customers, staff, or others in attendance will not become infected with COVID-19.
In light of the ongoing spread of COVID-19, individuals who fall within any of the categories below should not engage in Beehive play, programs, events, etc. By attending a Beehive event, you certify that you do not fall into any of the following categories: 1. Individuals who currently or within the past fourteen (14) days have experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath among others; 2. Individuals who have traveled at any point in the past fourteen (14) days either internationally or to a community in the U.S. that has experienced or is experiencing sustained community spread of COVID-19; or 3. Individuals who believe that they may have been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for their treatment.
ASSUMPTION OF THE RISK. I acknowledge and understand the following: 1. Participation includes possible exposure to and illness from infectious diseases including but not limited to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; 2. I knowingly and freely assume all such risks related to illness and infectious diseases, such as COVID-19, even if arising from the negligence or fault of the Released Parties; and 3. I hereby knowingly assume the risk of injury, harm and loss associated with the Activity, including any injury, harm and loss caused by the negligence, fault or conduct of any kind on the part of the Released Parties. Further, I hereby acknowledge and agree that: I received a copy of and agree to strictly comply (and cause my children to comply) with all Beehive’s rules and regulations for using the facility. I further agree to conduct myself (and cause my children to conduct themselves) in a controlled and reasonable manner at all times and refrain from using any equipment in a manner inconsistent with its intended design and purpose. I agree that Beehive may revoke or terminate my use of the facility at any time if I or my children violate any of its rules, regulations, or policies
I acknowledge that Beehive will not supervise my children at the facility and that I am solely responsible for doing so.
I agree to ensure that my child takes off all potentially unsafe clothing items including, but not limited to, hood and/ or neck drawstrings before playing on any equipment.
I agree to be solely responsible for the safety and well-being of my children and myself. I understand that Beehive does not provide any supervision, instruction, or assistance for using of the facility.
I understand and agree that Beehive is not responsible for any property that is lost, stolen, or damaged while in, on, or about the facility.
I represent and warrant that both I and the children maintain the skills and physical ability to properly and safely use the facilities and that Beehive has not and will not perform any type of screening to verify the foregoing.
I hereby waive any protections afforded by any statute or law in any jurisdiction whose purpose and/or effect is to provide that this waiver is invalid, limited or inapplicable and therefore I am releasing unknown future claims. Notwithstanding the above, if any claim is allowed for any reason I agree that my sole remedy is to enter into mutually binding arbitration in Pennsylvania where each party shall agree to select an arbitrator of their choosing and they will in turn agree on a neutral arbitrator in accordance with standard arbitration procedures.
I hereby consent to receive, and consent to the receipt by the children of emergency first aid which may be deemed advisable in the event of injury, accident, and/or illness related to the activities at the facility. If emergency first aid is rendered, I understand that I am waiving any and all claims resulting from the first aid and all the terms and provisions of this agreement remain in full force and effect under those circumstances.
I hereby give permission to Beehive to use any photos or videotape material taken of myself or my child/children. The photos and videotape material will only be used for advertising purposes online or in print. as with all advertising consent, I may at any time withdraw permission for photos or video footage of me and my child/children to be used in this advertising campaign.
