Skateboard Coaching Waiver and Release of Liability

This Waiver and Release of Liability (“Agreement”) is entered into as a condition of participation in private skateboard coaching, lessons, and related activities conducted by or on behalf of Utah1on1Skate & Coaches. 

By signing this Agreement, I acknowledge, accept, and expressly assume the risks inherent in skateboarding, skate coaching, and related activities, whether conducted at public skateparks, private facilities, or any other location. These risks include, but are not limited to, serious injury, paralysis, property damage, or death, resulting from falls, collisions with objects or other participants, equipment malfunction, adverse surface conditions, weather, or the acts or omissions of others. I understand and agree that these risks are unavoidable and are part of the very nature of the activity. I freely and voluntarily accept and assume all such risks, both known and unknown, and understand that participation is entirely of my own free will.

In consideration of being allowed to participate, I, for myself, my heirs, executors, administrators, assigns, and personal representatives, hereby fully release, discharge, and hold harmless Utah1on1Skate, its owners, officers, agents, contractors, employees, representatives, volunteers, successors, and assigns (the

“Released Parties”) from any and all claims, demands, liabilities, suits, damages, losses, costs, or expenses, including those arising out of the ordinary negligence of the Released Parties, that may result from or be in any way related to my participation or the participation of my child in coaching sessions, lessons, training, contests, media production, or other related activities. This release extends to claims for personal injury, illness, disability, property damage, and wrongful death.

I further agree to indemnify, defend, and hold harmless the Released Parties against any and all claims, causes of action, damages, judgments, costs, or expenses, including attorney’s fees, which may in any way arise from my participation or my child’s participation. I acknowledge that I am solely responsible for any medical, hospital, or related expenses that may result from injury or illness incurred during or in connection with participation, and I authorize emergency medical treatment if deemed necessary.

I acknowledge and agree that skateboarding involves physical exertion and requires the use of proper equipment, including helmets and pads, and that it is my responsibility, or the responsibility of my child, to participate with appropriate safety equipment and in a safe manner. I further acknowledge that the Coach/Business is not responsible for supervising the overall safety of the facility where the activity occurs, whether public or private, and that participation takes place in facilities not owned or controlled by the Released Parties.

I grant to Utah1on1Skate the irrevocable right and permission to photograph, record, and/or video me or my child during participation, and to use such photographs, images, likenesses, and recordings for promotional, marketing, advertising, social media, educational, or other lawful purposes, in any form of media now known or later developed, without compensation. I waive any right to inspect, approve, or claim ownership in the finished product or its use, and I release the Released Parties from any liability arising from such use.

If the participant is a minor under the age of eighteen (18), I, the undersigned parent or legal guardian, represent that I have legal authority to sign on their behalf. I consent to their participation in skateboard coaching and activities, accept and assume all risks on their behalf, and agree to all terms of this Agreement as though the minor themselves had executed it.

I understand that this Agreement is intended to be as broad and inclusive as permitted by the laws of the State of Utah (or the state in which the activities occur). If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. I acknowledge that I have read this Agreement, fully understand its terms, and sign it freely and voluntarily with the intent to be legally bound.
