We are pleased to offer you a temporary position with O2 White Studio. Each shift will be paid directly to you from the office at $45-48 per hour depending upon experience at the end of each day/week. A minimum of four hours is expected of each shift. Do not leave a shift early before notifying O2 White Studio, via call or text, and confirming the leave with the Dentist or Office Manager. Please provide all documentation and complete required paperwork needed to the office in which you are assigned for tax purposes. O2 White Studio is not responsible for tracking your pay or hours, nor do we provide any workers comp or liability insurance. 

Please provide the following documentations to O2 White Studio prior to applying for any shifts:

*Copy of resume

*Emergency Contact name and number

*Copy of drivers license

*Copy of x-ray certification

*Copy of Hygiene License

*Copy of infiltration license, if you have one

*A copy of liability insurance, if you have it

*Copy of nitrous certification

*Copy of CPR card

*Any professional references you may have

*Signed copy of this contract with O2 White Studio

*Any offices you prefer not to work at

*How you heard about O2 White Studio

All cancellations must be placed no later than 48 hours prior to the shift. Any cancellations or no shows made less than 24 hours by the Hygienist without a valid reason or emergency will be subject to discharge from O2 White Studio. Should the practice cancel the shift less than 24 hours, they are still required to pay you for the scheduled time frame reserved.

All communications are to go through O2 White Studio, not the office. If extended days are needed from you, please advise O2 White Studio immediately in order to book the shift(s). If you are interested or offered a permanent position with the office that is to be organized through O2 White Studio first.
