I have voluntarily elected to allow my child(ren) to participate in playing on any of the water slide(s) and/or kiddie pools and/or bounce houses located at Calvary Community church, and I fully understand that this involves jumping, climbing and sliding on air filled entertainment that contain water and that there are health and safety risks associated with this type of activity. I, therefore, assume all risk of injury, illness and/or death associated with this and I will hold harmless Calvary Community Church, Manteca, CA, or anyone affiliated with them for any circumstances related to water slide and kiddie pool injuries or deaths.the body of your waiver here.
