Participant Agreement, Assumption of Risk, Release of Liability, Medical Authorization & Media Release
Effective Beginning with the 2026–2027 Dance Season and All Future Participation

WELCOME

Welcome to IV League Studios! Our mission is to provide exceptional dance and performing arts education in a safe, positive, creative, and professional environment where students of all ages are encouraged to grow as artists and individuals. Please read this Agreement carefully before signing.

1. PARTIES

This Agreement is entered into between the participant (or parent/legal guardian if under 18) and IV League Studios, including IV League Dance, LLC, IV League Theatre Academy, IV League Music, Performance Lab, Agoura Hills Dance & Performing Arts Center, LLC, RSS Management, Inc., and all affiliated entities, owners, instructors, employees, contractors, volunteers, guest artists, sponsors, landlords, successors, and assigns (collectively, 'IV League Studios').

2. ACTIVITIES COVERED

This Agreement applies to all current and future IV League Studios programs including dance, theatre, acting, music, camps, workshops, master classes, private lessons, birthday parties, competitions, conventions, rehearsals, performances, productions, auditions, photoshoots, video shoots, studio rentals, off-site events, travel, fitness, conditioning, acro, tumbling, partnering, lifts, and any future services or events.

3. TERM

This Agreement remains effective from the date signed until replaced or revoked in writing. It applies to all participation regardless of season. IV League Studios generally operates from mid-August through mid-June with performances, productions, photoshoots, video shoots, camps and special events continuing through the end of June and throughout the year.

4. ASSUMPTION OF RISK

I understand participation involves inherent risks including slips, falls, strains, sprains, fractures, concussions, illness, disability and death. I voluntarily assume all known and unknown risks associated with participation.

5. PHYSICAL CONTACT

I understand instructors may use appropriate physical contact for spotting, safety, partnering, lifts and technical corrections.

6. MEDICAL FITNESS

I certify I (or my child) am physically able to participate and have disclosed any medical conditions affecting participation.

7. EMERGENCY MEDICAL AUTHORIZATION

I authorize IV League Studios to obtain emergency medical treatment if necessary and accept responsibility for all related expenses.

8. RELEASE OF LIABILITY

To the fullest extent permitted by California law, I release and waive claims against IV League Studios arising from participation, including claims for ordinary negligence.

9. INDEMNIFICATION

I agree to defend, indemnify and hold harmless IV League Studios from claims arising from my or my child's participation.

10. PERSONAL PROPERTY

IV League Studios is not responsible for lost, stolen or damaged personal property.

11. STUDIO PROPERTY

Participants agree to respect studio property and accept responsibility for intentional or reckless damage.

12. COMPETITIONS & OFF-SITE EVENTS

This Agreement applies equally to competitions, conventions, performances, travel, rehearsals and off-site activities.

13. THIRD-PARTY EVENTS

When outside organizations rent or use IV League Studios, the studio may only be providing the facility.

14. COMMUNICABLE ILLNESS

Participation carries inherent risk of exposure to communicable illnesses. Participants should not attend while contagious.

15. MEDIA RELEASE

I grant IV League Studios the perpetual right to photograph, film, livestream, record, edit, publish and use my or my child's name, likeness, image, voice and performances for marketing, educational, promotional and archival purposes without compensation.

16. AUDIENCE RECORDING

I understand audience members may photograph or record performances and IV League Studios cannot control subsequent distribution.

17. CODE OF CONDUCT

Participants and parents agree to behave respectfully. Harassment, bullying, threats or disruptive behavior may result in dismissal without refund.

18. SEVERABILITY

If any provision is unenforceable, the remainder remains in effect.

19. GOVERNING LAW

California law governs this Agreement.

20. DISPUTE RESOLUTION

Disputes will first be mediated and, if unresolved, submitted to binding arbitration in Ventura County, California.

21. ELECTRONIC SIGNATURE

Electronic signatures are legally binding.

22. ACKNOWLEDGMENT

I acknowledge I have read, understood and voluntarily agree to this Agreement and, if signing for a minor, have authority to do so.

I understand that tuition, billing, attendance, recital, competition, and studio operational policies are governed by a separate Studio Policies & Enrollment Agreement.
