Student Acknowledgment, Statement and Liability Release Agreement

NZ SAFETY, LLC - Less Lethal and/or Lethal Firearms Course: Student Acknowledgment, Statement and Liability Release Agreement hereby release NZ Safety, LLC, its Officers, Directors, Members, Instructors, associated persons and their Agents, Employees, and all other organizations affiliated with this course from any liability, loss, damage costs, claims and/or causes of action, including but not limited to all bodily and personal injuries and death and property damage, arising out of my participation in the less lethal and /or lethal firearms training course taught at by NZ Safety, LLC, it being specifically understood that said course(s) include(s) the operation and use by the undersigned and others of less lethal and/or lethal firearms, training tools and ammunition.  Furthermore, I fully release NZ Safety, LLC, its Officers, Directors, Members, Instructors, and their Agents, Employees, associated persons and all other organizations affiliated with this course from any liability, loss, damage, costs, claims and/or causes of action, including but not limited to all bodily and personal injuries and death and property damage to myself and any others, arising out of my application, practice implementation, demonstration, and/or use of any of the academic training, skills or instructional material presented during, or ancillary to, the course of my instruction. This release is given on behalf of myself, my heirs, successors, executors, administrators and assigns. I understand that there are inherent and other risks involved in the use of firearms training, training tools, guns and ammunition in shooting, and I freely and voluntarily assume and accept those risks .I agree to abide by all safety rules required by NZ Safety, LLC. I understand that my instruction may be terminated at anytime, and my fee forfeited, if my cooperation is deemed to be unsatisfactory by the instructor(s) or my behavior or actions are deemed to be a risk to others or myself. I understand that the safe and proper use of firearms and training tools requires a positive attitude based on maturity, responsibility, ethics and respect. I understand that my instruction may be terminated at anytime, and my fee forfeited, if in the opinion of the instructor(s), I fail to demonstrate the proper attitude and knowledge necessary for the safe and proper use of firearms and/or training tools. I understand that this/these course(s) present(s) general information for ACADEMIC purposes only, regarding firearm safety and self-defense and that it/they is/are not intended to be comprehensive in any way nor is/are it/they intended as legal advice. I understand that the laws governing firearm ownership, possession and self-defense are subject to frequent changes through legislation and court interpretation and that nothing in this course shall be taken as a definitive guide to the laws in any jurisdiction. For any legal questions, interpretations or advice, I will consult with a knowledgeable attorney. I certify that I am at least 18 years of age (or if a minor, under the age of 18, have parental/guardian permission, attested below), and have never been convicted of a felony. Furthermore, I certify that I have never been convicted of ANY violation of New York law pertaining to domestic violence and have never been institutionalized for any mental disorder.  I also certify that I am currently under no restriction, order or involved in ANY pending litigation that may preclude me from possessing a firearm under New York State law.  

The applicable laws of New York State govern this agreement. If any part of this agreement is determined to be unenforceable, all other parts shall be given full force and effect. I, the undersigned, have read and understand this Firearms Safety Course Student Statement and Liability Release Agreement and fully agree to all of its terms
