It's My Estuary Day 2025 Waiver 

2025 WAIVER AND RELEASE OF LIABILITY

It's My Estuary Day 2025 is a FREE event organized by New York State Marine Education Association (NYSMEA), John Dewey High School, City Parks Foundation, Partnerships for Parks, New York Aquarium, and Billion Oyster Project. I am the participant and/or parent/guardian of the participant named below. 

I hereby acknowledge that the information provided on this application is correct and I verify that the participant named below has permission to participate in It's My Estuary Day, in Kaiser Park, Brooklyn. Due to the COVID-19 pandemic, the event will uphold safety protocols to ensure the safety of all participants. I understand that participating in public activities at this time incurs a risk of coming in contact with the virus. I assume the risk of personal injury or property damage that may occur to me or that my child/ward may sustain. I waive any and all rights and claims against the City Parks Foundation, New York State Marine Education Association, Partnership for Parks, the New York City Department of Parks & Recreation, and any of our sponsors, and/or their employees for any injury and/or property damage suffered while participating in the program. I release the sponsors from any liabilities, damages, and expenses whatsoever that may be incurred as a result of my participation or my child/ward’s participation in this event.

I grant permission to City Parks Foundation, Billion Oyster Project, the NY Aquarium and NYSMEA, to use any photographs, motion pictures, recordings or any other record of this event for promotional, educational or legitimate public use (including commercial use by one or more of our sponsors) without compensation. This includes all forms of media, be it photography or digital video.

I HAVE READ AND UNDERSTOOD THIS WAIVER AND RELEASE. I CERTIFY THAT I AM PHYSICALLY AND MENTALLY FIT TO TAKE PART IN ALL PARTICIPANT ACTIVITIES, UNDERSTAND THE NATURE OF THE RISKS THAT MAY ARISE FROM SUCH ACTIVITIES, AND VOLUNTARILY ASSUME ALL SUCH RISKS.
